摘要
目的观察超声睫状体成形术(UCP)治疗新生血管性青光眼(NVG)短期疗效。方法前瞻性病例对照研究。在2018年1月至2020年5月期间,对我院接受UCP治疗的14例(14只眼)NVG患者(UCP组)和接受睫状体冷冻治疗的11例(11只眼)NVG患者(冷冻组),进行了为期6个月的随访观察,分别记录患者眼压、手术成功率、疼痛等级评分和并发症发生情况,以对手术效果进行评估。结果术前UCP组和冷冻组患眼的平均眼压分别为(42.00±6.41)mmHg和(42.55±6.35)mmHg。随访6个月后,UCP组患眼的平均眼压为(25.93±4.84)mmHg,冷冻组患眼的平均眼压为(24.45±6.86)mmHg,差异均无统计学意义(均为P>0.05);两组患眼的手术成功率分别为57.14%(8/14)和54.55%(6/11),两组间差异无统计学意义(均为P>0.05)。UCP组术后各个时间点的疼痛等级评分均较术前明显下降(均为P<0.05),冷冻组术后1d的疼痛等级评分较术前无明显差异(P>0.05),余术后各时间点的疼痛等级评分均较术前明显下降(均为P<0.05)。UCP组3只眼(21.43%)出现并发症,冷冻组8只眼(72.73%)出现并发症。结论UCP治疗NVG安全性高,降眼压效果明显,并可显著缓解患者疼痛。
Objective To observe the short-term efficacy of ultrasonic cycloplasty(UCP)in the treatment of neovascular glaucoma(NVG).Methods Prospective case-control studies were used.From January 2018 to May 2020,14 NVG patients receiving UCP(UCP group)and 11 NVG patients receiving cyclocryotherapy(cyclocryotherapy group)in our hospital were followed up for 6 months,and intraocular pressure,surgical success rate,pain rating and complications were recorded,respectively,to evaluate the surgical effect.Results The mean intraocular pressure was(42.00±6.41)mmHg and(42.55±6.35)mmHg in the UCP group and the cyclocryotherapy group,respectively.After 6 months of follow-up,the mean IOP of the UCP group was(25.93±4.84)mmHg,and that of the cyclocryotherapy group was(24.45±6.86)mmHg,with no statistically significant difference(all P>0.05).The operative success rate of the two groups was 57.14%(8/14)and 54.55%(6/11),respectively,and the difference between the two groups was not statistically significant(both P>0.05).Except that the pain grade score of the freezing group was higher one day after surgery,the pain grade score of the two groups was significantly lower than that before surgery at other time points(both P<0.05).Complications occurred in 3 eyes(21.43%)in UCP group and 8 eyes(72.73%)in cyclocryotherapy group.Conclusions UCP is safe in treating NVG,has obvious effect in lowering IOP,and can significantly relieve the pain of patients.
