丹参酮联合多西环素治疗寻常痤疮的临床疗效及对TNF-α的影响

Research on Clinical Efficacy of Tanshinone Combined with Doxycycline in The Treatment of Acne Vulgaris and Its Effect on TNF-α

目的评估丹参酮联合多西环素治疗寻常痤疮的临床疗效以及对血清中肿瘤坏死因子-α(TNF-α)表达的影响。方法将80例寻常痤疮患者随机分为对照组39例与治疗组41例,对照组给予多西环素(100 mg po bid),治疗组采用丹参酮(4粒po tid)联合多西环素(用法用量同前),治疗4周后对两组的临床疗效进行评价,检测两组患者治疗前后血清TNF-α的表达水平,并与健康组TNF-α水平比较,及评估不良反应发生情况。结果治疗组总有效率97.6%明显高于对照组总有效率82.1%(P<0.05),两组患者均未见明显不良反应。两组患者血清TNF-α的表达水平较健康组明显升高(P<0.000 5),治疗组治疗后TNF-α水平较治疗前明显降低(P<0.000 5),且低于对照组(P<0.000 5)。结论丹参酮能显著降低血清TNF-α水平,有效抑制炎症进展,联合多西环素方案治疗寻常痤疮的临床疗效更确切,安全可靠。

OBJECTIVE To evaluate the clinical efficacy of tanshinone combined with doxycycline in the treatment of acne vulgaris and the effect on the expression of serum tumor necrosis factor-α( TNF-α). METHODS 80 cases of acne vulgaris were randomly divided into the control group( n = 39) and the treatment group( n = 41). The control group was treated with doxycycline( 100 mg po bid),and the treatment group was treated with tanshinone( 4 capsules po tid) combining with doxycycline( usage and dosage as before). After 4 weeks of treatment,the clinical efficacy of the control and treatment groups were evaluated. The serum TNF-αexpression level was measured in both groups of patients before and after treatment and assessed the occurrence of adverse reaction in control and treatment groups comparing with those in the normal group. RESULTS The total effective rate of the treatment group was97.6%. The total effective rate of the control group was 82.1%. The difference of clinical efficacy between the two groups was statistically significant( P<0.05). No obvious adverse reactions occurred in both groups of patients. The expression level of serum TNF-α in the two groups were significantly higher than that in the normal group( P<0.000 5). The TNF-α level after treatment in the treatment group was significantly lower than before treatment( P < 0. 000 5),and lower than that in the control group( P < 0. 0005). CONCLUSION Tanshinone can significantly reduce the expression level of TNF-α in serum and effectively inhibit the progress of inflammation. The efficacy of tanshinone combining with doxycycline is more definite,safe and reliable than that before in the treatment of acne vulgaris.
