摘要
目的探讨锥形束CT在无牙颌患者种植义齿修复中的应用价值。方法13例单颌牙列缺失患者颌骨做CBCT扫描,采集相关数据,然后进行图像三维重建,对种植部位和解剖结构的关系、牙槽骨骨质及骨量进行分析判断,利用CBCT模拟患者最佳的种植部位、角度,结合种植导板软件辅助制作种植导板,在导板引导下开展种植手术,术后定期进行随访。结果患者均行固定式种植修复,共植入种植体93个,种植直径3.6~5.0mm,长度10~12mm;术后7d内所有患者未见牙龈肿胀等情况,切口愈合良好,术后3个月种植体和骨组织良好结合,术后6个月及1年随访发现,所有患者咬合状态无异常,咀嚼功能正常,且患者对美观度表示满意。结论在无牙颌患者种植义齿修复中应用CBCT,可提供牙槽骨骨质与骨量等数据信息,有效帮助医师准确预估种植体的植入方向、角度,进而能科学安全地完全种植手术。
Objective To evaluate the application value of cone beam computed tomography(CBCT)in implant denture restoration in edentulous jaw patients.Methods Thirteen patients with unimaxillary edentulous jaw were scanned by CBCT,and the related data were collected.Then three-dimensional image reconstruction was performed.The relationship between implant position and anatomical structure,alveolar bone and bone mass were analyzed and judged.CBCT was used to simulate the best implant position and angle of patients.With the help of planting guide plate software,planting guide plate was made and planting operation was carried out under the guidance of guide plate.The patients were followed up regularly after operation.Results All patients underwent fixed implant prosthesis,93 implants were implanted,the diameter of implants was 3.6~5.0 mm and the length was10~12 mm.No gingival swelling was found in all the patients within 7 d after operation.The incision healed well.The implant and bone tissue were well combined in 3 months after operation.Follow-up of 6 months and 1 year after operation showed that all patients had normal occlusal status,normal masticatory function and satisfactory aesthetics.Conclusion CBCT can provide the data of alveolar bone quality and bone mass in edentulous patients.It can also help physicians accurately predict the direction and angle of implant implant,and then complete implantation operation scientifically and safely.
