摘要
目的测定儿童骨龄(bone age,BA)和体重指数(body mass index,BMI),分析BA与BMI的相关性。方法纳入366例于2019年4~8月至笔者医院儿童保健门诊就诊的儿童,采用康娃婴幼儿身高体重仪和儿童身高体重测量仪HW-2000测量儿童的身高(身长)及体重,并计算BMI;根据左手腕部X线正位片,采用TW3法判定BA。采用分组、分层及偏相关分析探讨儿童BA与BMI之间的相关性。结果366例儿童年龄为8.62±2.74岁,29.5%的儿童(108例)BA落后,17.5%(64例)BA超前,53.0%(194例)BA正常。不同性别、年龄、不同BMI分组的BA/CA、BA分布比较,差异均有统计学意义(P=0.000),女童BA超前率高于男童,≥8且<10岁儿童的BA超前率高于其余3组,超重或肥胖组的BA超前率高于消瘦组和正常组,差异均有统计学意义(P<0.05),消瘦组和正常组的BA分布比较,差异无统计学意义(P>0.05)。按性别、年龄分层分析,男童和女童的BMI3组BA均值、BA/CA、BA分布比较,差异均有统计学意义(P<0.05),男童中超重或肥胖组BA超前率显著高于消瘦组和正常组,女童中超重或肥胖组BA落后率显著低于消瘦组和正常组;≥6且<8岁、≥8且<10岁、≥10且<16岁儿童BMI 3组的BA均值比较以及≥8且<10岁、≥10且<16岁儿童的BA/CA、BA分布比较,差异均有统计学意义(P<0.05),≥8且<10岁、≥10且<16岁儿童超重或肥胖组的BA均值、BA/CA、BA超前率均显著高于其余两组。控制性别、年龄变量后,偏相关分析显示,BA与BMI呈正相关(r=0.345,P=0.000)。结论儿童BA和BMI之间呈正相关,8岁以上的超重或肥胖的男女童均更容易出现骨龄超前。
Objective To analyze the correlation between bone age(BA)and body mass index(BMI)in children.Methods A total of 366 children who attended the child health clinic of our hospital from April 2019 to August 2019 were included in the study.The height or length and weight of the children were measured by Kangwa infant height and weight meter and child height and weight meter HW-2000,then BMI was calculated.BA was determined by TW3 method according to the left wrist X-ray orthotopic result.Grouping,stratification and partial correlation analysis were used to explore the correlation between BA and BMI in children.Results The age of 366 children was 8.62±2.74 years.29.5%of children(108 cases)had backward BA,17.5%(64 cases)had advanced BA,and 53.0%(194 cases)had normal BA.The ratio of BA/CA and BA distribution differences in different gender,age,BMI groups were statistically significant(P=0.000).The rate of advanced BA in girls was higher than that in boys.The rate of advanced BA in the 8~year old group was higher than that in the other three groups.The rate of advanced BA in the overweight or obese group was higher than that in the emaciated and normal groups,and all the differences were statistically significant(P<0.05).BA distribution difference between the emaciated and the normal group was no statistically significant.According to the stratification analysis of gender and age,the mean BA,BA/CA ratio and BA distribution difference in the three different BMI groups of boys and girls were statistically significant(P<0.05).The rate of advanced BA in the overweight or obese boys was significantly higher than that of the emaciated and the normal groups,while the rate of backward BA in the overweight or obese girls was significantly lower than that of the emaciated and the normal groups.The difference of the mean BA in the three BMI groups of children aged 6~,8~and 10~years and the difference of BA/CA ratio and BA distribution in children aged 8~and 10~years were statistically significant(P<0.05).The mean BA,BA/CA ratio and BA distribution in overweight or obese children aged 8~and 10~years were significantly higher than those in the other two BMI groups.After controlling for gender and age variables,partial correlation analysis showed that BA was positively correlated with BMI(r=0.345,P=0.000).Conclusion There is a positive correlation between BA and BMI in children,and overweight or obese boys and girls over 8 years old are more likely to have advanced bone age.
