基于循证理论的中西医结合护理模式对血液透析肾病患者的影响

Effect of Integrated Chinese and Western Medicine Nursing Mode Based on Evidence-based Theory on Nephropathy Patients Undergoing Hemodialysis

目的:探讨基于循证理论的中西医结合护理模式对血液透析肾病患者的影响。方法:选取2018年1月1日~2020年11月30日行血液透析的186例肾病患者为研究对象,按照随机数字法分为对照组和观察组各93例。对照组给予常规护理,观察组在常规护理基础上给予基于循证理论的中西医结合护理模式。比较两组护理前后肾功能(包括24 h尿蛋白、尿素氮、血肌酐)、心理状况[采用焦虑自评量表(SAS)和抑郁自评量表(SDS)]、睡眠质量[采用匹兹堡睡眠质量指数(PSQI)]、生活质量[采用肾病生活质量评分表1.3版(KDQOL-SF 1.3)],比较两组护理满意度。结果:护理后,观察组24 h尿蛋白、尿素氮、血肌酐低于对照组(P<0.01),SAS评分、SDS评分及PSQI各方面评分均低于对照组(P<0.01);护理后,观察组KDQOL-S F1.3各维度评分高于对照组(P<0.01);观察组护理态度、护理沟通、健康教育满意度得分高于对照组(P<0.01)。结论:对血液透析肾病患者实施基于循证理论的中西医结合护理模式,其临床效果好,可改善患者肾功能、心理状况、睡眠质量及生活质量,同时可提升护理满意度。

Objective:To explore the effect of integrated Chinese and western medicine nursing mode based on evidence-based theory on nephropathy patients undergoing hemodialysis.Methods:A total of 186 nephropathy patients who underwent hemodialysis during January 1,2018 to November 30,2020 were enrolled as subjects.According to random number table method,they were divided into the control group and the observation group,with 93 cases in each group.The control group was given routine nursing,while the observation group was given integrated Chinese and western medicine nursing based on evidence-based theory.The renal function(including the 24 h urine protein,urea nitrogen,and serum creatinine),psychological status by the self-assessment scale on anxiety(SAS)and depression self rating scale(SDS),sleep quality with Pittsburgh sleep quality index(PSQI),quality of life with Version 1.3 with kidney disease quality of life scale(KDQOL-SF 1.3)and nursing satisfaction were compared between the two groups.Results:After nursing,scores of 24h urine protein,blood urea nitrogen and serum creatinine in observation group were lower in the observation group than those in the control group(P<0.01),scores of anxiety,depression and PSQI were lower than those in the control group(P<0.01),scores of KDQOL-SF 1.3 were higher than those in the control group(P<0.01),and scores of satisfaction with nursing attitude,nursing communication and health education were higher than those in the control group(P<0.01).Conclusion:The clinical effect of integrated Chinese and western medicine nursing based on evidence-based theory is good on nephropathy patients undergoing hemodialysis.It can improve renal function,psychological status,sleep quality and quality of life,and increase nursing satisfaction.
