预防性淋巴结清扫在矿区甲状腺乳头状微小癌手术治疗中的价值

Value of prophylactic lymph node dissection in the surgical treatment of papillary thyroid microcarcinoma in mining area

目的:探讨预防性淋巴结清扫在矿区甲状腺乳头状微小癌(PTMC)手术治疗中的价值。方法:选取2018-2020年行甲状腺切除手术的矿区PTMC患者60例,分为两组,各30例。观察组行预防性淋巴结清扫;对照组未行预防性淋巴结清扫。比较两组血清甲状旁腺素(PTH)和钙(Ca)指标水平、术后并发症、复发率及远处转移率。结果:观察组术后3 d血清PTH、Ca水平均低于对照组,术后远处转移率也低于对照组,差异均有统计学意义(P<0.05);两组术后并发症发生率及复发率比较,差异无统计学意义(P>0.05)。结论:预防性中央区淋巴结清扫可降低矿区PTMC患者的远处转移,且术后并发症较少。

Objective:To explore the value of prophylactic lymph node dissection in the surgical treatment of papillary thyroid microcarcinoma(PTMC)in mining area.Methods:During 2018-2020,60 cases of PTMC patients in mining area who underwent thyroidectomy were selected and divided into two groups with 30 patients in each group.The observation group received prophylactic lymph node dissection.No prophylactic lymph node dissection was performed in the control group.The levels of serum parathyroid hormone(PTH)and calcium(CA),postoperative complications,recurrence rate and distant metastasis rate were compared between the two groups.Results:The levels of serum PTH and Ca in the observation group were lower than the control group 3 days after operation,and the distant metastasis rate was also lower than the control group,the difference was statistically significant(P<0.05).The incidence and recurrence rate of postoperative complications between the two groups were compared,with the difference was not statistically significant(P>0.05).Conclusion:Prophylactic lymph node dissection can reduce the distant metastasis of PTMC patients in mining area,and there are fewer postoperative complications.
