淋巴细胞/单核细胞比值与急性脑梗死关系的临床研究

Relationship between lymphocyte/monocyte ratio and acute cerebral infarction

目的:探讨淋巴细胞/单核细胞比值(LMR)与急性脑梗死(ACI)的关系。方法:选取2019年初发ACI患者120例,作为观察组;另选取同期体检健康者100例,作为对照组。记录并分析两组研究对象的一般临床资料;根据Rankin改良量表(mRS)评分标准,将观察组患者进一步分为预后良好组及预后不良组。采用二元Logistic回归分析影响脑梗死预后的相关因素,采用受试者工作曲线(ROC)判断LMR对脑梗死预后的鉴别能力。结果:观察组患者高血压及糖尿病患病率、血脂水平、各类白细胞数值、中性粒细胞/淋巴细胞比值(NLR)水平均明显高于对照组,而LMR水平明显低于对照组,差异均有统计学意义(P<0.05)。Logistic回归分析表明中性粒细胞(NE)、淋巴细胞(LY)、NLR、LMR等指标与ACI预后不良相关(P<0.05);ROC曲线表明,LMR<2.56时,是判断ACI患者预后的独立危险因素。结论:低LMR对ACI患者预后不良具有一定的预测价值。

Objective:To explore the relationship between lymphocyte/monocyte ratio(LMR)and acute cerebral infarction(ACI).Methods:In 2019,120 patients with initial ACI were selected as the observation group.Another 100 healthy people in the same period were selected as the control group.We recorded and analyzed the general clinical data of the two groups.According to the Rankin modified scale(MRS)scoring standard,the patients in the observation group were further divided into the good prognosis group and the poor prognosis group.Binary Logistic regression was used to analyze the related factors affecting the prognosis of cerebral infarction,receiver operating curve(ROC)was used to judge the ability of LMR to distinguish the prognosis of cerebral infarction.Results:In the observation group,the prevalence of hypertension and diabetes,blood lipid levels,various leukocyte values and NLR levels were significantly higher than those in the control group,while the LMR level was significantly lower than that in the control group,the difference was statistically significant(P<0.05).Logistic regression analysis shows that neutrophils(NE),lymphocytes(LY),NLR,LMR and other indicators are associated with poor prognosis of ACI(P<0.05).ROC curve showed that LMR<2.56 was an independent risk factor for prognosis of ACI patients.Conclusion:Low LMR can predict the poor prognosis of ACI patients.
