剖宫产后瘢痕早期妊娠诊断中经阴道彩色多普勒超声的应用价值分析

Application value of transvaginal color Doppler ultrasound in the diagnosis of scar early pregnancy after cesarean section

目的:研究剖宫产后瘢痕早期妊娠诊断中经阴道彩色多普勒超声的应用价值。方法:2017年1月-2019年12月收治剖宫产后瘢痕早期妊娠患者86例,所有患者均行经阴道彩色多普勒超声检测,将检测结果与术后病理检查结果进行对比,观察并记录诊断符合率,并分析声像图特征。结果:经阴道彩色多普勒超声诊断的符合率为94.19%,漏诊率为5.81%;经阴道彩色多普勒超声声像图显示,妊娠囊完全处于宫腔内或宫腔中下段位置6例,妊娠囊为团块型19例,妊娠囊位于子宫瘢痕处56例。结论:剖宫产后瘢痕早期妊娠诊断中经阴道彩色多普勒超声的应用具有重要意义,子宫瘢痕处妊娠囊周边检测到低阻滋养细胞血流信号是瘢痕妊娠的重要声像学特征之一,有利于提升剖宫产后瘢痕早期妊娠诊断准确率。

Objective:To explore the application value of transvaginal color Doppler ultrasound in the diagnosis of scar early pregnancy after cesarean section.Methods:From January 2017 to December 2019,86 patients with scar early pregnancy after cesarean section were selected,all patients were detected by transvaginal color Doppler ultrasound.The detection results were compared with the postoperative pathological examination results,the diagnostic coincidence rate was observed and recorded,and the sonographic characteristics were analyzed.Results:The coincidence rate of transvaginal color Doppler ultrasound was 94.19%,and the missed diagnosis rate was 5.81%.Transvaginal color Doppler ultrasound showed that the gestational sac was completely located in the uterine cavity or the middle and lower part of the uterine cavity in 6 cases,the gestational sac was mass type in 19 cases,and the gestational sac was located in the uterine scar in 56 cases.Conclusion:It is of great significance of transvaginal color Doppler ultrasound in the diagnosis of scar early pregnancy after cesarean section,the detection of low impedance trophoblast blood flow around the gestational sac at uterine scar is one of the important sonographic features of scar pregnancy,it is helpful to improve the accuracy of scar early pregnancy after cesarean section.
