内镜下电凝、钛夹联合注射肾上腺素治疗消化性溃疡出血的临床疗效

Clinical efficacy of endoscopic electrocoagulation and titanium clip combined with epinephrine injection in the treatment of peptic ulcer bleeding

目的:比较内镜下电凝联合注射肾上腺素及钛夹联合注射肾上腺素治疗消化性溃疡出血的临床疗效。方法:2018年3月-2019年4月收治消化性溃疡出血患者105例,随机分为三组,各35例。对照组采用内镜下注射肾上腺素治疗消化性溃疡出血,电凝组采用内镜下电凝联合注射肾上腺素治疗,钛夹组采用钛夹联合注射肾上腺素治疗,比较三组治疗效果。结果:电凝组和钛夹组止血成功率高于对照组,再出血率低于对照组,差异有统计学意义(P<0.05)。电凝组和钛夹组在首次止血成功、止血失败、止血成功、再出血方面比较,差异无统计学意义(P>0.05)。结论:内镜下电凝、钛夹联合注射肾上腺素治疗消化性溃疡出血的临床疗效是一致的,同样提高了溃疡出血的治愈率,减少了创伤。

Objective:To compare the clinical efficacy of endoscopic electrocoagulation combined with epinephrine injection and titanium clip combined with epinephrine injection in the treatment of peptic ulcer bleeding.Methods:From March 2018 to April 2019,105 patients with peptic ulcer bleeding were selected,they were randomly divided into the three groups with 35 cased in each group.The control group was treated with endoscopic injection of epinephrine.The electrocoagulation group was treated with endoscopic electrocoagulation combined with epinephrine injection.The titanium clip group was treated with titanium clip combined with epinephrine injection.The therapeutic effects of the three groups were compared.Results:The success rate of hemostasis in electrocoagulation group and titanium clip group was higher than that in the control group,and the proportion of rebleeding in electrocoagulation group and titanium clip group was lower than that in control group,the difference was statistically significant(P<0.05).There was no significant difference in success of first hemostasis,failure of hemostasis,success of hemostasis and rebleeding between the electrocoagulation group and the titanium clip group(P>0.05).Conclusion:The clinical efficacy of endoscopic electrocoagulation and titanium clip combined with epinephrine injection in the treatment of peptic ulcer bleeding is consistent,which can also improve the cure rate of ulcer bleeding and reduce trauma.
