摘要
目的探讨消岩汤联合吉西他滨/卡铂(GC)化学药物治疗晚期非小细胞肺癌(NSCLC)的预测价值。方法回顾性分析65例接受消岩汤联合GC化学药物治疗的晚期NSCLC患者临床资料,比较有效组和无效组治疗前和后磁共振扩散加权成像(DWI)表观扩散系数(ADC_(pre)和ADC_(post))及ADC变化率(ΔADC),分析ADC_(pre)、ADC_(post)及ΔADC与肿瘤消退率的相关性及对疗效的预测价值。结果消岩汤联合GC化学药物治疗有效率为58.46%。治疗前DWI均表现为高信号,ADC_(pre)较低;治疗后DWI以等信号为主,ADC_(post)较高。肿瘤消退率与ADC_(pre)负相关,与ADC_(post)和ΔADC正相关(P<0.05)。ADC_(pre)预测疗效的灵敏度、特异度、准确度和AUC分别为71.05%、81.48%、75.38%、0.789;ADC_(post)为89.47%、66.67%、80.00%、0.810;ΔADC为81.58%、85.19%、83.08%、0.842。结论晚期NSCLC患者ADC_(pre)、ADC_(post)及ΔADC与肿瘤消退率关系密切,DWI能够较好预测消岩汤联合GC化学药物治疗的临床疗效。
Objective To explore the predictive value for the efficacy of Xiaoyan decoction combined with gemcitabine/carboplatin(GC)chemotherapy in the treatment of advanced non-small cell lung cancer(NSCLC).MethodsThe clinical data from 65 patients with advanced NSCLC who were treated with Xiaoyan decoction and GC chemotherapy were analyzed retrospectively.The apparent diffusion coefficient before and after treatment(ADC_(pre)and ADC_(post))of magnetic resonance diffusion-weighted imaging(DWI)and ADC change rate(ΔADC)were compared between the effective group and the ineffective group.The correlation of ADC_(pre)and ADC_(post)andΔADC with tumor regression rate and their predictive value for curative effect were analyzed.ResultsThe effective rate of Xiaoyan decoction combined with GC chemotherapy was 58.46%.Before treatment,DWI showed a high signal,ADC_(pre)was low,and DWI was mainly equal signal and ADC_(post)was high after treatment.The tumor regression rate was negatively correlated with ADC_(pre),and positively correlated with ADC_(post)andΔADC(P<0.05).The sensitivity,specificity,accuracy andAUC of ADC_(pre)for efficacy prediction were 71.05%,81.48%,75.38%,0.789,respectively,and ADC_(post)was 89.47%,66.67%,80.00%,0.810,andΔADC was 81.58%,85.19%,83.08%,0.842.Conclusions ADC_(pre),ADC_(post)andΔADC are closely related to tumor regression rate in advanced NSCLC patients.DWI can predict the clinical efficacy of Xiaoyan decoction combined with GC chemotherapy.
