医院护士主导的慢性病团队管理模式对冠心病患者疾病管理能力及生活质量的影响 被引量：10

摘要 目的探讨医院护士主导的慢性病团队管理模式对冠心病患者疾病管理能力及生活质量的影响。方法选取本院辖区内2个社区冠心病患者120例,采用随机抽样法将2个社区分为观察组、对照组各60例。对照组给予常规随访服务,观察组开展以医院护士为主导的慢性病团队管理。比较两组干预前及干预6个月后,评价患者的冠心病自我管理量表(CSMS)与西雅图心绞痛问卷(SAQ)评分,统计两组主要不良心血管事件(MACE)发生率及再入院率。结果干预6个月后,观察组的CSMS、SAQ量表评分均升高且高于对照组,观察组MACE发生率及再住院率均低于对照组,差异均有统计学意义(P<0.05)。结论医院护士主导的慢性病团队管理模式能够提高CHD患者的疾病管理能力,减少心血管不良事件发生率,并改善患者的生活质量。
