独活寄生汤加减结合短刺加电针夹脊穴治疗寒湿痹阻型腰椎间盘突出症的临床研究

Clinical study of modified Duhuo Jisheng decoction combined with short needling plus electroacupuncture at Jiaji point in treating lumbar disc herniation of cold dampness obstruction type

目的研究寒湿痹阻型腰椎间盘突出症患者使用独活寄生汤加减结合短刺加电针夹脊穴治疗的效果。方法120例寒湿痹阻型腰椎间盘突出症患者,随机分为对照组和研究组,每组60例。对照组口服布洛芬缓释胶囊结合短刺加电针夹脊穴治疗,研究组在对照组的基础上联合独活寄生汤加减治疗。比较两组患者治疗前后的视觉模拟评分法(VAS)评分、日本骨科协会评估治疗(JOA)评分、腰椎活动度、Oswestry功能障碍指数量表(ODI)评分及治疗效果。结果治疗后,两组患者的VAS评分均低于本组治疗前,JOA评分均高于本组治疗前,且研究组患者的VAS评分(3.19±0.66)分低于对照组的(4.82±0.85)分,JOA评分(20.81±4.19)分高于对照组的(16.26±4.37)分,差异具有统计学意义(P<0.05)。治疗后,两组患者的腰椎活动度均大于本组治疗前,ODI评分均低于本组治疗前,且研究组患者的腰椎活动度(27.77±8.19)°大于对照组的(21.64±7.47)°,ODI评分(17.17±7.85)分低于对照组的(21.85±8.44)分,差异具有统计学意义(P<0.05)。研究组患者的治疗总有效率为95.00%,高于对照组的83.33%,差异具有统计学意义(P<0.05)。结论独活寄生汤加减结合短刺加电针夹脊穴治疗寒湿痹阻型腰椎间盘突出症,可明显缓解腰腿疼痛症状,促进腰椎功能恢复,提高整体疗效。

Objective To study the effect of modified Duhuo Jisheng decoction combined with short needling plus electroacupuncture at Jiaji point in treating lumbar disc herniation of cold dampness obstruction type.Methods A total of 120 patients with lumbar disc herniation of cold dampness obstruction type were randomly divided into control group and research group,with 60 cases in each group.The control group was treated with oral ibuprofen sustained-release capsule combined with short needling and electroacupuncture at Jiaji points,and the research group was treated with modified Duhuo Jisheng Decoction on the basis of the control group.The visual analogue scale(VAS)score,Japanese Orthopedic Association(JOA)score,lumbar range of motion and Oswestry dysfunction index(ODI)score before and after treatment,and therapeutic effects were compared between the two groups.Results After treatment,the VAS scores of the two groups were lower than those before treatment,and JOA scores were higher than those before treatment;the VAS score(3.19±0.66)points of the research group was lower than(4.82±0.85)points of the control group and JOA score(20.81±4.19)points was higher than(16.26±4.37)points of the control group.All the difference was statistically significant(P<0.05).After treatment,the lumbar range of motion of the two groups were greater than those before treatment,and ODI scores were lower than those before treatment;the lumbar range of motion(27.77±8.19)°of the research group was greater than(21.64±7.47)°of the control group,and ODI score(17.17±7.85)points was lower than(21.85±8.44)points of the control group.All the difference was statistically significant(P<0.05).The total effective rate of the research group was 95.00%,which was higher than 83.33%of the control group,and the difference was statistically significant(P<0.05).Conclusion Combination of Duhuo Jisheng decoction and short needling and electroacupuncture at Jiaji point for patients with lumbar disc herniation of cold dampness obstruction type can obviously relieve the symptoms of lumbocrural pain,promote the recovery of lumbar function and improve the overall efficacy.
