小切口甲状腺腺叶切除术治疗甲状腺癌的临床效果及对创伤程度的影响

The clinical effect of small incision thyroid lobectomy in the treatment of thyroid cancer and its influence on the degree of trauma

目的探讨小切口甲状腺腺叶切除术治疗甲状腺癌的临床效果及对创伤程度的影响。方法抽取2016年1月~2020年1月收治的30例甲状腺癌患者,根据手术方式分为观察组及对照组各15例,对照组行常规甲状腺腺叶切除术(切口4~6cm),观察组行小切口甲状腺腺叶切除术(切口2cm),对两组手术结果进行评价。结果两组引流时间及引流量无显著差异(P>0.05),观察组切口长度、术中出血量与对照组有显著差异(P<0.05);观察组心率、血红蛋白、心脏征象、创口污染、白细胞计数、Glasgow昏迷各项创伤评分均较对照组低(P<0.05);观察组并发症发生率为6.67%,较对照组26.67%低(P<0.05)。结论小切口甲状腺腺叶切除术在甲状腺癌中应用价值教高,可缩短切口长度,减少术中出血及术后并发症,减轻手术创伤程度,值得应用。

Objective To explore the clinical effect of small incision thyroid lobectomy in the treatment of thyroid cancer and its influence on the degree of trauma.Methods A sample of 30 thyroid cancer patients admitted from January 2016 to January 2020 were divided into observation group and control group with 15 cases each according to the operation method.the control group underwent conventional thyroid lobectomy(incision 4~6cm),and the observation group underwent small incision thyroid lobectomy(incision 2cm).the surgical results of the two groups were evaluated.Results There were no significant difference in drainage time and drainage volume between the two groups(P>0.05).There were significant differences in incision length and intraoperative blood loss between the observation group and the control group(P<0.05);The observation group’s heart rate,hemoglobin,heart signs,wound contamination,white blood cell count,Glasgow coma,all trauma scores were lower than those of the control group(P<0.05);The complication rate in the observation group was 6.67%,which was lower than 26.67%in the control group(P<0.05).Conclusion Small incision thyroid lobectomy has high application value in thyroid cancer.It can shorten the length of the incision,reduce intraoperative bleeding and postoperative complications,and reduce the degree of surgical trauma.It is worth applying.
