摘要
目的探讨关节镜下前交叉韧带重建术不同股骨隧道钻取方式的短期疗效。方法选取2017年1月至2018年12月在徐闻县人民医院就诊的前交叉韧带损伤患者157例,根据不同股骨隧道钻取方式进行分组,观察组78例采用由外向内隧道钻取,对照组79例采用前内侧入路进行隧道钻取,比较两组患者手术时间、股骨隧道钻取的长度、手术前后IKDC及Lysholm评分等。结果观察组患者股骨隧道的长度(34.47±2.53)mm明显低于对照组患者股骨隧道的长度(37.21±2.14)mm,差异具有统计学意义(P<0.05)。两组手术时间比较,差异无统计学意义(P>0.05)。术前两组患者IKDC评分、Lysholm评分比较,差异无统计学意义(P>0.05),两组患者术后IKDC评分、Lysholm评分分别均显著高于术前,差异有统计学意义(P<0.05)。结论关节镜下前交叉韧带重建术中,由外向内隧道钻取的方式能够明显缩短股骨隧道钻取的长度,临床上可推广应用。
Objective To explore the short-term effect of different femoral tunnel drilling in arthroscopic anterior cruciate ligament(ACL)reconstruction.Methods A total of 157 patients with ACL injury admitted to People’s Hospital of Xuwen County from January 2017 to December 2018 were selected.According to different drilling methods of femoral tunnel,78 cases in the observation group were drilled from outside to inside,79 cases in the control group were drilled through the anteromedial approach.The surgery time,length of femoral tunnel drilling,IKDC and Lysholm scores before and after operation were compared between the two groups.Results The length of femoral tunnel in the observation group was(34.47±2.53)mm,which was significantly lower than that in the control group(37.21±2.14)mm,and the difference was statistically significant(P<0.05).There was no significant difference in the surgery time between the two groups(P>0.05).There was no significant difference in IKDC score and Lysholm score of patients between the two groups before operation(P>0.05).The postoperative IKDC score and Lysholm score of the two groups were significantly higher than those before operation,and the difference was statistically significant(P<0.05).Conclusion In arthroscopic ACL reconstruction,drilling from outside to inside can obviously shorten the length of femoral tunnel drilling,which can be widely used in clinic.
