摘要 肾缺血再灌注损伤(RIRI)多继发于肾脏手术、器官移植、严重创伤和休克等,是急性肾损伤高病死率的主要原因,目前尚无特效治疗方法。磷脂酰肌醇-3-激酶/蛋白激酶B(PI3K/Akt)信号通路是调节细胞生长、增殖、代谢和迁移的重要通路,可调节下游的多种信号分子,并通过调节自噬、抑制凋亡、减轻氧化应激、调节内皮功能损伤等在RIRI保护中发挥重要作用。进一步研究PI3K/Akt通路在RIRI中的作用和机制以及该通路对RIRI远期并发症(如纤维化)的影响,对于探索RIRI可能的发病机制及进一步治疗均具有重要意义。 Renal ischemia-reperfusion injury(RIRI)usually occurs in those with renal surgeries,organ transplantation,severe trauma and shock etc.,which is the main cause of high mortality of acute kidney injury,and there is no specific effective treatment currently available.The phosphatidylinositol-3-kinase/protein kinase B(PI3 K/Akt)signaling pathway is an important pathway regulating cell growth,proliferation,metabolism and migration,which can regulate a variety of downstream signaling molecules,and plays an important role in the protection of RIRI by regulating autophagy,inhibiting apoptosis,alleviating oxidative stress and regulating endothelial function damage.Further study on the role and mechanism of PI3 K/Akt pathway in RIRI and its effect on the long-term complications(such as fibrosis)of RIRI is of great significance to explore the possible pathogenesis and further treatment of RIRI.
