摘要
目的:探究无创正压通气治疗慢性阻塞性肺疾病急性加重期合并重症呼吸衰竭的临床效果。方法:以2018年7月~2019年6月作为研究时段,选择此期间本院收治的慢性阻塞性肺疾病急性加重期合并重症呼吸衰竭患者,共78例,按照入院先后顺序编号,前39例设定为对照组,给予常规治疗,后39例设定为观察组,在对照组基础上进行无创正压通气治疗,比较两组治疗效果和血气指标变化情况。结果:观察组总有效率为94.87%,高于对照组76.92%,P<0.05;两组治疗前PaO_(2)、PaCO_(2)、SaO_(2)组间比较,差异无统计学意义,P>0.05,治疗后PaCO_(2)较治疗前降低,PaO_(2)、SaO_(2)较治疗前升高,P<0.05,组间比较观察组变化幅度更大,P<0.05。结论:无创正压通气治疗慢性阻塞性肺疾病急性加重期合并重症呼吸衰竭的临床效果显著,可有效改善血气指标,值得临床借鉴。
Objective:To explore the clinical effect of noninvasive positive pressure ventilation in the treatment of acute exacerbation of chronic obstructive pulmonary disease with severe respiratory failure.Methods:A total of 78 patients with acute exacerbation of chronic obstructive pulmonary disease complicated with severe respiratory failure in our hospital from July 2018 to June 2019 were selected and numbered according to the order of admission.The first 39 cases were set as the control group and was given conventional treatment,and the last 39 cases were set as the experimental group and was treated with noninvasive positive pressure ventilation on the basis of the control group.The clinical effect and blood gas index of the two groups were compared.Results:The total effective rate of the experimental group was 94.87%,which was higher than that of the control group(76.92%),P<0.05;there was no significant difference in PaO_(2),PaCO_(2)and SaO_(2)between the two groups before treatment,P>0.05;after treatment,PaCO_(2)of the two groups was lower than that before treatment,PaO_(2)and SaO_(2)were higher than that before treatment,P<0.05,and the change range of the experimental group was greater than that of the control group,P<0.05.Conclusion:The clinical effect of noninvasive positive pressure ventilation in the treatment of acute exacerbation of chronic obstructive pulmonary disease with severe respiratory failure is significant,which can effectively improve the blood gas index,which is worthy of clinical reference.
