X线检查联合CT检查在诊断老年慢性肺炎中的应用效果

Application of X-ray Examination Combined with CT Examination in the Diagnosis of Senile Chronic Pneumonia

目的:在对社区老年慢性肺炎(Chronic pneumonia,CP)患者进行诊断过程中,对比单纯应用X线检查和X线+CT联合检查的效果,判断联合检查的应用价值。方法:选取社区2019年全年进行CP诊断的老年患者64例进行研究,对其应用X线与X线+CT检查,并以临床综合检查作为金标准,对不同检查方法的准确率和影像学特点进行比较分析。结果:联合组检查的准确率高达95.31%,而单一X线检查仅达到81.25%,联合检查高于单一检查,P<0.05,有统计学意义。X线显示病灶边界并不清晰,直径在1和7cm之间,主要形状为椭圆、类圆和不规则形状。其中,不清晰病灶边界患者有21例;椭圆形状24例、18例类圆和10例不规则形状。CT显示病灶边界并不清晰,密集度不高,轮廓有光整也有轮廓不齐,肺部增粗血管影和毛玻璃阴影以及胸膜增厚。其中,轮廓不齐23例,类圆形25例,增粗血管阴影、毛玻璃阴影以及胸膜增厚各有19例、16例、11例。结论:X线检查是临床诊断老年CP的常用诊断方法,其具有一定的应用价值,但相比于单纯应用X线,联合应用诊断的准确率更高、效果更好,所以,可以根据临床实际情况对诊断方法进行选择,酌情选择联合检查。

Objective:In the diagnosis process of elderly patients with chronic pneumonia(CP)in the community,to compare the effect of simple X-ray examination and X-ray+CT combined examination,and to judge the application value of combined examination.Methods:64 elderly patients diagnosed with CP in the community throughout 2019 were selected for the study,and X-ray and X-ray+CT were applied to them,and clinical comprehensive examination was used as the gold standard to evaluate the accuracy and imaging of different examination methods.The characteristics are compared and analyzed.Results:The accuracy rate of the combined group examination was as high as 95.31%,while the single X-ray examination only reached 81.25%.The combined examination was higher than the single examination,P<0.05,which was statistically significant.The X-ray showed that the boundary of the lesion was not clear,the diameter was between 1 and 7 cm,and the main shape was ellipse,quasi-circle and irregular shape.Among them,21 patients had unclear lesion boundaries;24 had elliptical shapes,18 had round-like shapes,and 10 had irregular shapes.CT showed that the boundary of the lesion was not clear,the density was not high,the outline was smooth and uneven,the lungs were thickened with vascular shadows and ground glass shadows,and the pleura was thickened.Among them,there were 23 cases with irregular contours,25 cases with round shapes,and 19 cases,16 cases,and 11 cases with thickened blood vessel shadows,ground glass shadows,and pleural thickening.Conclusion:X-ray examination is a common diagnostic method for clinical diagnosis of CP in the elderly.It has certain application value.However,compared with the simple application of X-ray,the combined application diagnosis has higher accuracy and better effect.Therefore,it can be based on clinical practice.Choose the diagnosis method according to the situation,and choose joint inspection as appropriate.
