50例意外胆囊癌的诊治和预后分析

Diagnosis,treatment and prognosis of unexpected gallbladder carcinoma:a report of 50 cases

目的总结意外胆囊癌的临床诊治经验,分析意外胆囊癌患者的预后。方法回顾性分析2005年1月至2015年12月间蚌埠医学院第一附属医院肝胆外科收治的50例意外胆囊癌患者的人口统计学特征、检查结果、病理结果、手术信息和预后情况。结果根治性手术、早期肿瘤对意外胆囊癌患者的预后有显著影响(P<0.05)。对T1b期患者,胆囊癌根治术生存率明显优于单纯胆囊切除术(P<0.05)。多因素分析显示T分期和根治性手术是影响预后的独立因素(P<0.05)。结论肿瘤的早期诊断和根治性治疗对意外胆囊癌患者的长期生存至关重要。为达到良好预后效果,建议T1b期患者行胆囊癌根治术。

Objective To summarize the clinical diagnosis and treatment experience of unexpected gallbladder cancer,and to analyze the prognosis of patients with unexpected gallbladder cancer.Methods The demographic characteristics,examination results,pathological results,surgical information and prognosis of 50 patients with unexpected gallbladder cancer admitted to the First Affiliated Hospital of Bengbu Medical College from January 2005 to December 2015 were retrospectively analyzed.Results Radical surgery and early tumors have a significant impact on the prognosis of patients with accidental gallbladder cancer(P<0.05).For patients with stage T1b,the survival rate of radical resection of gallbladder cancer was significantly better than simple cholecystectomy(P<0.05).Multivariate analysis showed that T staging and radical surgery were independent factors affecting the prognosis(P<0.05).Conclusion Early diagnosis and radical treatment of tumors are critical to the long-term survival of patients with accidental gallbladder cancer.In order to achieve a good prognosis,radical resection of gallbladder cancer would be better for patients with stage T1b.
