引气归元灸联合空气波压力治疗老年静脉血栓栓塞症患者的效果及对凝血因子的影响

Effect of Yinqi Guiyuan Moxibustion combined with air wave pressure in treating elderly patients with venous thromboembolism and its influence on coagulation factors

目的探讨引气归元灸联合空气波压力治疗老年静脉血栓栓塞症(VTE)的效果。方法选取2018年2月—2020年3月于广东省中医院老年内科住院的100例老年VTE患者,按照随机数字表法分为对照组和观察组,各50例。对照组予空气波压力治疗及常规治疗,观察组在对照组基础上联合引气归元灸治疗,治疗时间为14 d。比较两组下肢深静脉血栓发生率、股静脉血流速度、血浆D-二聚体、纤维蛋白原、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)和不良症状情况。结果观察组治疗后下肢深静脉血栓发生率低于对照组,差异有统计学意义(P<0.05)。治疗后,两组股深静脉血流速度快于治疗前,且观察组快于对照组,差异有统计学意义(P<0.05)。治疗后,两组D-二聚体和纤维蛋白原水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗后,两组PT、APTT和TT均长于治疗前,且观察组长于对照组,差异有统计学意义(P<0.05)。两组深静脉血栓阳性症状比较,差异无统计学意义(P>0.05)。结论引气归元灸联合空气压力波治疗老年VTE的效果显著,能够改善静脉血流速度和血液高凝淤滞状态,明显降低老年内科住院患者肢体深静脉血栓的发生率。

Objective To explore the effect of Yinqi Guiyuan Moxibustion combined with air wave pressure in treating elderly patients with venous thromboembolism(VTE).Methods One hundred elderly patients with VTE hospitalized in Department of Geriatrics of Guangdong Provincial Hospital of Traditional Chinese Medicine from February 2018 to March 2020 were selected and divided into control group and observation group according to random number table method,with 50 cases in each group.Control group was given air wave pressure treatment and conventional treatment,while observation group was given Yinqi Guiyuan Moxibustion treatment on the basis of control group for 14 days.The incidence of deep venous thrombosis,femoral venous blood flow velocity,plasma D-dimer,fibrinogen,prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT)and adverse symptoms were compared between two groups.Results After treatment,the incidence of deep venous thrombosis in observation group was lower than that in control group,and the difference was statistically significant(P<0.05).After treatment,the blood flow velocity of deep femoral vein in two groups was faster than that before treatment,and observation group was faster than that of control group,the differences were statistically significant(P<0.05).After treatment,the levels of D-dimer and fibrinogen in two groups were lower than those before treatment,and those in observation group were lower than those in control group,the differences were statistically significant(P<0.05).After treatment,PT,APTT and TT of two groups were longer than those before treatment,and observation group was longer than that of control group,the differences were statistically significant(P<0.05).There was no significant difference in the positive symptoms of deep venous thrombosis between two groups(P>0.05).Conclusion The effect of Yinqi Guiyuan Moxibustion combined with air wave pressure in treating elderly patients with VTE is significant,which can improve the venous blood flow velocity and blood stasis state,and significantly reduce the incidence of limb deep venous thrombosis in elderly medical inpatients.
