术前口服麦芽糊精果糖液对胃癌患者围术期胃肠功能的影响

Effect of oral maltodextrin fructose solution on gastrointestinal function in patients with gastric cancer during perioperative period

目的2组探讨术前口服麦芽糊精果糖液对胃癌患者围术期胃肠功能的影响。方法选取2017年1月—2019年6月连云港市第二人民医院胃肠外科收治的胃癌患者80例,采用随机数字表法分为试验组和对照组各40例,分别予以术前口服麦芽糊精果糖液和常规禁饮禁食。比较2组患者胃排空情况,术前、术后血浆内毒素浓度、肿瘤坏死因子及血浆胰岛素样生长因子Ⅰ的浓度,术后肠功能恢复。结果2组患者胃排空情况差异无统计学意义(P>0.05)。试验组患者术后第1,3天血浆内毒素浓度明显低于对照组,差异有统计学意义(P<0.05),术前、术后第7天血浆内毒素浓度无统计学差异(P>0.05)。试验组肿瘤坏死因子α浓度术后第1,3天明显低于对照组,差异有统计学意义(P<0.05),2组术前、术后第7天肿瘤坏死因子α浓度无统计学差异(P>0.05)。试验组血浆胰岛素样生长因子Ⅰ浓度术后第1,3天明显高于对照组,差异有统计学意义(P<0.05),2组术前、术后第7天血浆胰岛素样生长因子Ⅰ浓度无统计学差异(P>0.05)。试验组患者排气时间、排便时间均短于对照组,差异有统计学意义(P<0.05)。结论术前口服麦芽糊精果糖液用于胃癌根治术患者安全可行,能促进术后肠功能恢复,值得临床推广。

Objective To investigate the effect of oral maltodextrin fructose solution on gastrointestinal function in patients with gastric cancer during perioperative period.Methods Selection 80 cases of gastric cancer,between January 2017 and June 2019 in our gastrointestinal surgery patients,through the random number table method into the group of patients can be divided into the experimental group and control group,the gastric emptying were observed,preoperative and postoperative serum endotoxin concentration,tumor necrosis factor and the concentration of plasma insulin-like growth factor I,postoperative intestinal function recovery.Results There was no significant difference in gastric emptying between the two groups(P>0.05).The plasma endotoxin concentration of the experimental group was significantly lower than that of the control group on the first and third day after surgery,with statistically significant difference(P<0.05),there was no statistically significant difference in plasma endotoxin concentration before and after surgery(P>0.05).The concentration of TNF in the experimental group was significantly lower than that in the control group on the first and third day after surgery,with statistically significant difference(P<0.05).There was no statistically significant difference in the concentration of tumor necrosis factor before and on the 7th day after surgery(P>0.05).The plasma concentration of insulin-like growth factor I in the experimental group was significantly higher than that in the control group on the 1st and 3rd day after surgery,with statistically significant difference(P<0.05),the plasma concentration of insulin-like growth factor I was not statistically significant before and 7 days after surgery(P>0.05).The exhaust time and defecation time of the experimental group were lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Preoperative oral maltodextrin fructose solution is safe and feasible in perioperative period for patients with radical gastrectomy,promotes the recovery of intestinal function and is worthy of clinical promotion.
