容积旋转调强放疗行同期推量治疗脑转移瘤的疗效分析

Analysis of the therapeutic effect of simultaneous plus volumetric modulated arc therapy in the treatment of brain metastatic tumors

目的:分析采用容积旋转调强技术同期推量(SIB-VMAT)治疗合并1~4个脑转移瘤患者的疗效及不良反应。方法:65例脑转移瘤患者随机分为同期推量组(34例)及序贯组(31例),同期推量组PGTV 50 Gy/20 f;序贯组PTV 40 Gy/20 f,全脑放疗结束后给予PGTV加量,10 Gy/5 f。比较两组的有效率、不良反应及1年生存率。结果:本研究显示,同期推量组有效率为88.2%,明显高于序贯组,且放疗不良反应两组间无差异,患者可耐受。结论:同期推量放疗是治疗1~4个脑转移瘤的有效手段。

Objective:To discuss the clinical curative effect and adverse effects of SIB-VMAT in treatment of patients with 1~4 brain metastases.Methods:65 cases of patients with brain metastasis in our hospital were randomly divided into two groups:Simultaneous integrated boost(34 cases)or sequential integrated boost(31 cases).The patients in the simultaneous integrated boost group were treated with VMAT.The whole of plan gross tumor volume dose was 50 Gy/20 f(5 times weekly).The sequential integrated boost group were treated of 40 Gy/20 f(5 times weekly),then add 10 Gy/5 f.The effective rate,adverse effects and one-year survival rate were compared between the two groups.Results:The effective rate of the simultaneous integrated boost group was 88.2%,significantly better than the another group.And no significant differences in the incidence of every adverse effects.Conclusion:Simultaneous plus volumetric modulated arc therapy is a safe and effective treatment for the patients with 1~4 brain metastases.
