宫颈上皮内瘤样病变患者组织中STAT3、EGFR表达变化及临床意义

Expression changes and clinical significance of STAT3 and EGFR in cervical intraepithelial neoplasia

目的探讨宫颈上皮内瘤样病变(CIN)患者组织中信号转导与转录因子(STAT3)、表皮生长因子受体(EGFR)的表达变化及临床意义。方法回顾性收集辽阳市中心医院2018年12月~2019年12月收治的260例CIN患者的临床资料,根据宫颈病变情况分为CINⅠ级组(轻度非典型增生,102例)、CINⅡ级组(中度非典型增生,89例)、CINⅢ级组(重度非典型增生和原位癌,69例)。比较三组的STAT3、EGFR表达水平,并绘制ROC曲线图分析STAT3、EGFR表达量及其联合检测诊断CIN程度的价值。结果Western blot检测结果显示,CINⅢ级组的STAT3、EGFR表达水平高于CINⅡ级组及CINⅠ级组,差异有统计学意义(P<0.05);ROC曲线图结果显示,STAT3、EGFR诊断CIN程度的AUC分别为0.807、0.835,具有一定诊断价值;联合检测的AUC为0.903,且其特异性、灵敏度均高于单一检测,提示联合检测诊断性能高。结论STAT3、EGFR表达量均与CIN程度有关,联合检测其水平具有较高的诊断价值,可早期预测临床宫颈癌病变。

Objective To investigate the expression changes and clinical significance of signal transducer and activator of transcription 3(STAT3)and epidermal growth factor receptor(EGFR)in patients with cervical intraepithelial neoplasia(CIN).Methods The clinical data of 260 patients with CIN admitted to Liaoyang Central Hospital from December 2018 to December 2019 were retrospectively collected.According to the cervical lesions,they were divided into the CIN classⅠgroup(mild atypical hyperplasia,102 cases),the CIN classⅡgroup(moderate atypical hyperplasia,89 cases)and the CIN classⅢgroup(severe atypical hyperplasia and carcinoma in situ,69 cases).The expression levels of STAT3 and EGFR were compared among the three groups,and the ROC curve was drawn to analyze the value of STAT3 and EGFR expression and their combined detection in the diagnosis of CIN.Results Western blot analysis showed that the expression levels of STAT3 and EGFR in the CIN classⅢgroup were higher than those in the CIN classⅡgroup and the CIN classⅠgroup and the differences were statistically significant(P<0.05).AUC of STAT3 and EGFR in diagnosing CIN was 0.807 and 0.835 respectively,which had certain diagnostic value.The AUC of combined detection was 0.903,and the specificity and sensitivity of combined detection were higher than those of single detection,which indicated that the diagnostic performance of combined detection was high.Conclusion The expression levels of STAT3 and EGFR are all related to the degree of cervical intraepithelial neoplasia.Combined detection of their levels has high diagnostic value and can be used for early prediction of clinical cervical cancer.
