失能老人照顾者照顾体验质性研究的Meta整合

Qualitative studies on experience of disabled elderly caregivers:a Meta⁃synthesis

目的:对失能老人照顾者照顾体验的质性研究进行Meta整合。方法:计算机检索PubMed、Web of Science、EMbase、The Cochrane Library、EBSCO、中国知网、万方数据库、维普数据库和超星发现系统,纳入关于失能老人照顾者照顾体验的质性研究,检索时限均为建库至2020年7月31日。运用澳大利亚JBI循证卫生保健中心质性研究质量评价标准(2016年)进行文献质量评价后,采用Meta整合方法对结果进行整合和分析。结果:共纳入13篇文献,提炼出73个结果,将结果整合归纳分类,最终形成7个类别和3个整合结果。整合结果1,照顾对个人及家庭的影响,包括照顾对个人自我实现的影响和照顾对家庭生活的影响2个类别;整合结果2,失能老人照顾者情绪体验,包括负性情绪体验和正性情绪体验2个类别;整合结果3,照顾者对支持的渴望,包括渴望医务人员指导、渴望政府的政策支持和渴望家人、朋友的理解支持3个类别。结论:现有证据表明,医务人员、政府和社区需充分关注失能老人照顾者的照顾感受和体验,给予相应的支持和指导,从而改善失能老人照顾者的身心状况,提高其生活质量。

Objective:To take a Meta⁃analysis for qualitative studies on nursing experience of disabled elderly caregivers.Methods:Qualitative studies on the nursing experience of disabled elderly caregivers were searched in electronic databases,including PubMed,Web of Science,EMbase,The Cochrane Library,EBSCO,CNKI,WanFang Database,VIP Database and ChaoXing Discovery System.The search time was from inception to July 31,2020.Quality of the literatures were evaluated according to JBI Critical Appraisal Checklist for Qualitative Research and then Meta⁃synthesis was performed.Results:Totally 13 studies were included and 73 research results were extracted.Finally seven categories and three integrated results were formed.Integrated result 1:The impacts of care on individuals and families,including the impact of care on personal self⁃actualized and the impact of care on family life.Integrated result 2:The emotional experience of caregivers of disabled elderly,including negative emotional experience and positive emotional experience.Integrated result 3:Caregiver's desire for support,including eagerness for guidance from medical staff,eagerness for government policy support and eagerness for understanding and support from family and friends.Conclusions:Current evidence showed that,medical staff,the government and the community need to pay full attention to the experience of caregivers of disabled elderly and to provide corresponding support and guidance,so as to improve the physical and mental condition of caregivers and improve their quality of life.
