孤立性肺结节CT征像在鉴别良恶性结节中的价值

The value of CT sign of solitary pulmonary nodule in distinguishing benign and malignant nodules

目的探讨孤立性肺结节的各种CT征像对良恶性结节鉴别的意义。方法选取41例术后病理结果的孤立性肺结节CT征像,依据病理结果把病例分成恶性结节组23例,良性结节组18例,对两组患者的性别、年龄及结节直径对比分析,对孤立性肺结节的毛刺征、分叶征、胸膜尾征、血管集束征、空泡征、磨玻璃结节及钙化等CT征像进行单一CT征像及组合CT征像对比分析。结果两组中患者性别差异无统计学意义,平均年龄差异有统计学意义(P=0.006),结节平均直径差异无统计学意义,单一CT征像比较,分叶征、血管集束征、磨玻璃结节及钙化差异有统计学意义(P值均<0.05),而毛刺征、胸膜尾征及空泡征差异无统计学意义(P值均>0.05),组合CT征像比较,毛刺征+分叶征、胸膜尾征+血管集束征、毛刺征+分叶征+胸膜尾征、毛刺征+分叶征+血管集束征、毛刺征+分叶征+胸膜尾征+血管集束征均差异有统计学意义(P值均<0.05),毛刺征+分叶征+空泡征差异无统计学意义(P值>0.05)。结论单一CT征像对孤立性肺结节的良恶性鉴别有一定的意义,而多种组合CT征像对孤立性肺结节的良恶性鉴别更有意义,诊断符合率明显提高。

Objective To explore the significance of various CT signs of solitary pulmonary nodules in the identification of benign and malignant nodules.Methods A retrospective analysis was performed on 41 CT signs of solitary pulmonary nodules with pathological results.The cases were divided into 23 cases of malignant nodules and 18 cases of benign nodules.And nodule diameter,single CT sign and combination of CT signs such as burr sign,lobulation sign,pleural tail sign,vascular bundle sign,vacuole sign,ground glass nodule and calcification were subjected to Comparative analysis.Results There was no statistically significant difference in gender between the two groups of patients,and there was a statistically significant difference in the mean age(P=0.006).There was no statistically significant difference in the average diameter of the nodules.Comparing the single CT signs,the lobular sign,vascular bundle sign,Ground-glass nodules and calcification were statistically significant(all P values were<0.05),while glitch signs,pleural tail signs,and vacuole signs were not statistically significant(all P values were greater than 0.05).Comparing the combined CT signs,the glitch signs+Foliar sign,pleural tail sign+vascular bundle sign,burr sign+lobular sign+pleural tail sign,burr sign+lobular sign+vascular bundle sign,burr sign+lobular sign+pleural tail sign+vascular bundle sign had no significance were statistical significance(all P values<0.05),no burr sign+leaf sign+vacuole sign(P value>0.05).Conclusion A single CT sign has certain significance for the identification of benign and malignant solitary pulmonary nodules,and multiple combined CT signs are more meaningful for the benign and malignant solitary pulmonary nodules,and the diagnostic coincidence rate is significantly improved.
