血小板/淋巴细胞比值与肺结核病情严重程度的相关性研究

Relationship between platelet and lymphocyte ratio and the severity of pulmonary tuberculosis

目的探讨血小板/淋巴细胞比值(PLR)与肺结核病情严重程度的相关性。方法选取120例肺结核患者,并将肺结核患者分为轻度组(n=46)、中度组(n=42)和重度组(n=32),另选取健康体检者126例为对照组,比较各组间PLR的差异;采用ROC曲线评估PLR对肺结核病情严重程度的预测价值。结果肺结核组患者PLR水平明显高于对照组(P <0.05);随着肺结核病情严重程度的增加,PLR水平逐渐升高,即重度组患者PLR水平高于中度组和轻度组(P <0.01),并且中度组患者PLR水平高于轻度组;有空洞组患者PLR水平明显高于无空洞组,上述差异均有统计学意义(P <0.01);ROC曲线分析提示PLR对肺结核病情严重程度和空洞型肺结核均有较好的预测价值,曲线下面积分别为0.782(95%CI:0.714~0.850,P <0.01)、0.760(95%CI:0.687~0.832,P <0.01)。结论 PLR升高与肺结核病情严重程度密切相关,可作为预测肺结核病情严重程度的一种临床指标。

Objective To explore the relationship between platelet and lymphocyte ratio( PLR) and the severity of pulmonary tuberculosis. Methods A total of 120 pulmonary tuberculosis patients were selected and divided into the mild group( n = 46),moderate group( n = 42) and severe group( n = 32),and 126 healthy people were selected as the control group. Differences of PLR among these three groups were compared. ROC curve was used to evaluate the predictive value of PLR for the severity of pulmonary tuberculosis. Results Patients with pulmonary tuberculosis had higher PLR level than that in control group( P <0. 05). PLR level in the severe group was higher than that in the moderate group and the mild group( P < 0. 01),and PLR level in the moderate group was higher than that in the mild group. Patients with cavitations had higher PLR level than that in patients without cavitations,with the differences statistically significant( P < 0. 01). ROC curve analysis indicated that PLR had a good predictive value for the severity of pulmonary tuberculosis severity and cavernous tuberculosis. The areas under the curve were 0. 782( 95% CI: 0. 714-0. 850,P < 0. 01) and 0. 760( 95% CI: 0. 687-0. 832,P < 0. 01). Conclusion PLR is closely related to the severity of pulmonary tuberculosis,which can be used as a clinical indicator to predict the severity of pulmonary tuberculosis.
