应用ROC曲线探讨血清甲状腺球蛋白抗体、抗甲状腺过氧化物酶抗体、促甲状腺素受体抗体在分化型甲状腺癌诊断中的临床价值 被引量:15

Clinical value of serum TGAb,TPOAb and TRAb in the diagnosis of differentiated thyroid cancer by ROC curve

摘要 目的探讨血清TGAb、Tpo Ab、TRAb在分化型甲状腺癌(DTC)鉴别及诊断中的临床应用价值。方法选取本院2019年1月-2020年1月分化型甲状腺癌患者120例为实验组及甲状腺良性病变患者120例为对照组,通过对血清甲状腺球蛋白抗体(TGAb)、抗甲状腺过氧化物酶抗体(TPOAb)及促甲状腺素受体抗体(TRAb)的水平进行检测,应用ROC曲线分析三者对DTC的诊断价值及鉴别意义。结果实验组的血清TGAb、Tpo Ab、TRAb水平明显高于对照组,差异有统计学意义(P <0.05)。ROC曲线结果显示,血清TGAb的曲线下面积(AUC)为0.763,TRAb的AUC为0.817,TPOAb的AUC为0.864(P <0.05),血清TGAb临界点为25.57 UI/ml,血清TRAb的临界点为3.56 UI/L,血清TPOAb的临界点为70.39 UI/ml。结论血清TGAb、TRAb、TPOAb具备诊断及鉴别DTC的临床价值。 Objective To investigate the clinical value of serum TGAb,TPOAb and TRAb in the differential diagnosis of differentiated thyroid cancer( DTC). Methods A total of 120 patients with differentiated thyroid cancer from January 2019 to January 2020 were selected as the experimental group and 120 patients with benign thyroid disease as the control group. The levels of serum thyroglobulin antibody( TGAb),antithyroid peroxidase antibody( TPOAb) and thyrotropin receptor antibody( TRAb)were detected,and ROC curve was used to analyze the diagnosis of DTC Value and identification significance. Results The levels of TGAb,TPOAb and TRAb in the experimental group were significantly higher than those in the control group,with the differences statistically significant( P < 0. 05). The results of ROC showed that AUC of serum TGAb was 0. 763,AUC of TRAb was 0. 817,AUC of TPOAb was 0. 864( P < 0. 05);critical point of serum TGAb was 25. 57 UI/ml,critical point of serum TRAb was 3. 56 UI/l,and critical point of serum TPOAb was 70. 39 UI/ml. Conclusion Serum TGAb,TRAb and TPOAb have clinical value in the diagnosis and differential diagnosis of DTC.
