自拟益气养阴通络汤联合西药治疗糖尿病周围神经病变患者的疗效

Study on the Therapeutic Effect of Self-made Yiqi Yangyin Tongluo Decoction Combined with Western Medicine on Patients with Diabetic Peripheral Neuropathy

目的探讨自拟益气养阴通络汤联合西药治疗糖尿病周围神经病变(DPN)患者的疗效。方法选取2017年7月至2019年3月于抚顺市中医院内分泌科诊治的90例糖尿病下肢周围神经病变患者作为研究对象,首先根据入院顺序进行编号,奇数列为对照组,偶数列为观察组,每组45例。对照组予以红外线照射双下肢治疗,配合静脉滴注灯盏花素注射液及α-硫辛酸注射液,观察组在对照组基础上予以自拟益气养阴通络汤口服治疗。比较两组患者多伦多临床评分系统(TCSS)评分、腓肠神经的感觉神经传导速度(SNCV)、腓总神经的运动神经传导速度(MNCV)、临床疗效、血液流变学指标、血小板最大聚集率。结果治疗后,两组神经反射、感觉功能、神经症状评分明显下降(P<0.05),且观察组神经反射、感觉功能、神经症状评分明显低于对照组(P<0.05);治疗后,两组腓肠神经SNCV、腓总神经MNCV显著提高(P<0.05),且观察组腓肠神经SNCV、腓总神经MNCV显著高于对照组(P<0.05);观察组治疗有效率(86.67%)明显高于对照组(68.89%)(P<0.05);治疗后,两组血液流变学指标、血小板最大聚集率显著下降(P<0.05),且观察组血液流变学指标、血小板最大聚集率显著低于对照组(P<0.05)。结论自拟益气养阴通络汤联合营养神经活血西药治疗DPN可明显提高疗效,有效改善血液高凝状态。

Objective To explore the clinical effect of Yiqi Yangyin Tongluo Decoction combined with Western medicine on patients with diabetic peripheral neuropath. Methods A total of 90 patients with diabetic peripheral neuropathy in the lower extremities diagnosed and treated in the Department of Endocrinology in Fushun Traditional Chinese Medicine Hospital were selected as the research objects and were numbered according to the order of admission. The odd number was the observation group and the even number was the control group. The control group was treated with infrared radiation treatment of both lower limbs, combined with intravenous drip of Breviscapine injection and α-lipoic acid injection. The observation group was treated with Yiqi Yangyin Tongluo Decoction plus infrared radiation treatment of both lower limbs. Toronto Clinical Scoring System(TCSS) scores, the sensory nerve conduction velocity(SNCV) of the sural nerve, the motor nerve conduction velocity(MNCV) of the common peroneal nerve, clinical efficacy, hemorheological indexes, and the maximum platelet aggregation rate between the 2 groups of patients were compared. Results After treatment, the sural nerve SNCV and common peroneal nerve MNCV in the 2 groups were significantly increased(P<0.05). The scores of nerve reflex, sensory function, and neurological symptoms in the observation group were significantly lower than those in the control group(P<0.05);after treatment, the sural nerve SNCV and common peroneal nerve MNCV in the 2 groups increased significantly(P<0.05), and the SNVC in the observation group and common peroneal nerve MNCV were significantly higher than those of the control group(P<0.05);the effective rate of treatment in the observation group(86.67%) was significantly higher than that of the control group(68.89%)(P<0.05);after treatment, the blood rheology of the two groups The academic indexes and the maximum platelet aggregation rate decreased significantly(P<0.05). After treatment, and the hemorheology indexes and the maximum platelet aggregation rate of the the 2 group were significantly lowered(P<0.05). Conclusion The self-made Yiqi Yangyin Tongluo Decoction combined with nutrient nerve and blood circulation Western medicine can significantly improve the curative effect and effectively improve the blood hypercoagulable state.
