乳腺癌根治术患者症状负担对其生活质量的影响

Correlation between symptom burden and quality of life in patients undergoing radical resection of breast cancer

目的探究乳腺癌根治术患者症状负担对其生活质量的影响。方法收集60例乳腺癌根治术患者临床资料,根据生命质量问卷-乳腺癌53项(QLQ-BR53)平均得分将其分为良好组(<66.3分,n=31)和欠佳组(>66.3分,n=29);比较两组患者症状负担(M.D.Anderson症状问卷,MDASI)、心理韧性(Connor-Davidson弹性量表,CD-RISC)获益感(疾病获益感量表,BFS)及应对方式(医学应对问卷,MCMQ)差异;通过Pearson相关系数模型分析各指标与生活质量的相关性,并借助二元Logistic回归模型分析指标是否能对患者生活质量产生独立影响。结果良好组患者症状负担明显低于欠佳组,而心理韧性、获益感、应对方式各项评分及总分均明显高于欠佳组(P<0.05);症状负担评分与生命质量评分表现为显著正相关性(r>0,P<0.05),而心理韧性、获益感、应对方式评分均与生命质量评分表现为显著负相关性(r<0,P<0.05);MDASI评分并非导致生活质量良好的独立影响因素(P>0.05),而心理韧性、获益感、应对方式均是导致生活质量良好的独立影响因素(P<0.05)。结论乳腺癌根治术患者症状负担与生活质量密切相关。

Objective To explore the effects of symptom burden on quality of life in patients undergoing radical resection of breast cancer.Methods The clinical data of 60 patients with radical resection of breast cancer were retrospectively analyzed.According to the average score of quality of life questionnaire-53 items for breast cancer(QLQ-BR53),they were divided into good group(QLQ-BR53<66.3 points,n=31)and poor group(QLQ-BR53>66.3 points,n=29).The symptom burden[MD Anderson Symptom Inventory(MDASI)],psychological resilience[Connor-Davidson Resilience Scale(CD-RISC)],benefit perception[Benefit Finding Scale(BFS)]and coping styles[Medical Coping Modes Questionnaire(MCMQ)were compared between the two groups.Pearson correlation coefficient model was used to analyze the correlation between each index and quality of life,and the binary Logistic regression model was used to analyze whether the indexes could have an independent impact on quality of life of patients.Results The MDASI in good group was significantly lower than that in poor group,and the scores of items and total score of CD-RISC,BFS and MCMQ were significantly higher than those in poor group(P<0.05).The MDASI score showed a significant positive correlation with QLQ-BR53 score(r>0,P<0.05),and the scores of CD-RISC,BFS and MCMQ showed a significant negative correlation with the QLQ-BR53 score(r<0,P<0.05).MDASI score was not an independent influencing factor for good quality of life(P>0.05),and CD-RISC,BFS and MCMQ were independent influencing factors for good quality of life(P<0.05).Conclusion The symptom burden of patients with radical resection of breast cancer is closely related to the quality of life,but the former does not affect the latter independently.It still needs to pay attention to the psychological environment construction of patients in clinical nursing.
