2型糖尿病中医证型与血尿酸、颈动脉粥样硬化的相关性研究

Correlation Between TCM Syndrome of Type 2 Diabetes Mellims and Serum Uric Acid,Carotid Atherosclerosis

目的:分析2型糖尿病(type 2 diabetes mellims,T2DM)患者中医证型与血尿酸水平、颈动脉粥样硬化的相关性。方法:对280例已确诊为T2DM患者的相关资料进行回顾分析和整理,记录一般情况及中医证型、空腹血糖(fasting plasma glucose,FPG)、糖化血红蛋白(hemoglobin A1C,HbA1c)、血尿酸(serum uric acid,SUA)、总胆固醇(total cholesterol,TC)、三酰甘油(triglyceride,TG)及颈动脉内膜厚度(intima media thickness,IMT),并比较分析各中医证型与上述指标的关系。结果:T2DM患者中医证型以气阴两虚证最多,证型呈从痰(湿)热互结证、热盛伤津证、气阴两虚证至阴阳两虚证的发展趋势;各证型的年龄、病程、HbA1c、SUA、TC、TG及IMT比较,阴阳两虚证均明显高于其他证型(P<0.05);T2DM合并高尿酸血症患者IMT增厚程度明显高于血尿酸正常患者(P<0.05),阴阳两虚证患者的IMT增厚值明显高于其他证型。结论:T2DM中医证型以气阴两虚证最多;阴阳两虚证的HbA1c、SUA、TC、TG及IMT均明显高于其他证型,高尿酸血症患者IMT增厚程度高于血尿酸正常患者。

Objective:To analyze the correlation between TCM syndrome of type 2 diabetes mellims(T2DM)and serum uric acid,carotid atherosclerosis.Methods:The data of 280 patients with T2DM were reviewed and analyzed.The general situation and TCM syndrome,fasting Plasma Glucose(FPG),hemoglobin A1 C(HbA1 c),serum uric acid(SUA),total cholesterol(TC),triglyceride(TG)and intima media thickness(intima)were recorded,and the relationship between TCM syndrome and the above indexes was compared.Results:The TCM syndrome of T2DM patients are mostly syndrome of deficiency of both qi and yin.The development trend of TCM syndrome is from syndrome of intermin-gled phlegm dampness and heat,syndrome of excessive heat injuring body fluid syndrome,syndrome of deficiency of both qi and yin to syndrome of deficiency of both yin and yang.The age,course,HbA1c,SUA,TC,TG and IMT of each syndrome were significantly higher than those of other syndromes(P<0.05);the IMT thickening degree of T2DM patients with hyperuricemia was significantly higher than that of normal patients with SUA(P<0.05),and the IMT thickening value of syndrome of deficiency of both yin and yang was significantly higher than other syndromes.Conclusion:The TCM syndrome of T2DM is syndrome of deficiency of both qi and yin;the HbA1c,SUA,TC,TG and IMT of syndrome of deficiency of both yin and yang are significantly higher than those of other syndromes;the IMT thickening degree of hyperuricemia patients is higher than that of patients with normal SUA.
