针灸联合肺功能康复早期介入对机械通气脑炎患儿功能恢复的影响

Effect of Early Intervention of Acupuncture Combined with Early Pulmonary Function Rehabilitation on Functional Recovery of Children with Mechanical Ventilation Encephalitis

目的:研究针灸联合早期肺功能康复介入对机械通气脑炎患儿功能恢复的影响。方法:将56例机械通气脑炎患儿随机分为对照组和治疗组,每组各28例,对照组给予常规方法治疗,治疗组在对照组治疗的基础上实施针灸联合早期肺功能康复介入治疗,比较两组患儿治疗前后心功能[左心室舒张末期内径(left ventricular end-diastolic diameter, LVEDD)、左心室收缩末期内径(left ventricular end-systolic diameter, LVESD)及左心室射血分数(left ventricular ejection fraction, LVEF)]、治疗后脱机率、肺功能[1 s用力呼气量(forced expiratory volume in one second, FEV1)、用力肺活量(forced vital capacity, FVC)、最大通气量(maximum volume, MVV)及1秒率(FEV1/FVC)]、运动功能评分[Fugl-Meyer运动功能评定量表(Fugl-Meyer assessment, FMA)]及肌张力评分[改良Ashworth量表(modified ashworth scale, MAS)]。结果:治疗后两组患儿的心功能指标LVEDD及LVESD较治疗前明显下降,LVEF较治疗前明显升高(P<0.05),治疗组FEV1、FVC、MVV、FEV1/FVC和FMA评分均较对照组明显升高,肌张力MAS评分较对照组明显降低,且治疗组优于对照组(P<0.05),治疗组临床有效率为92.00%,脱机率为92%,均明显优于对照组(P<0.05)。结论:针灸联合早期肺功能康复介入治疗机械通气脑炎患儿疗效显著,可有效提高机械通气的脱机率,改善患儿心肺功能、运动功能,提高生活质量及预后。

Objective: To study the effect of acupuncture combined with early pulmonary function rehabilitation intervention on the functional recovery of children with mechanical ventilation encephalitis.Methods: 56 children with mechanical ventilation encephalitis were randomly divided into control group and treatment group, 28 cases in each group.The control group was given conventional treatment, while the treatment group was given acupuncture combined with early pulmonary function rehabilitation intervention on the basis of the control group.The cardiac function of the child[left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD) and left ventricular ejection fraction(LVEF)],weaning rate after treatment, lung function [forced expiratory volume in one second(FEV1),forced vital capacity(FVC),maximum volume(MVV) and one second rate(FEV1/FVC)],motor function score [Fugl-Meyer assessment(FMA)] and muscle tone score [modified ashworth scale(MAS)] were compared before and after treatment.Results: After treatment, the cardiac function indexes LVEDD and LVESD of the two groups of children were significantly lower than before treatment, and LVEF was significantly higher than before treatment(P<0.05).The FEV1,FVC,MVV,FEV1/FVC and FMA scores of the treatment group were higher than those of the control group.Muscle tension MAS score was significantly lower than that of the control group, and the treatment group was better than the control group(P<0.05).The total clinical effective rate in the treatment group was 92.00%,and the weaning rate was 92%,which were significantly better than those in the control group(P<0.05).Conclusion: Acupuncture combined with early pulmonary function rehabilitation intervention has a significant effect on children with mechanical ventilation encephalitis, which can effectively improve the weaning efficiency of mechanical ventilation, improve the children′s cardiopulmonary function, motor function, and improve the quality of life and prognosis.
