ER、PR、Her-2、Ki-67在2型糖尿病合并乳腺癌患者肿瘤组织中的表达分析

摘要 目的探讨ER、PR、Her-2、Ki-67在肿瘤组织中的表达对于2型糖尿病合并乳腺癌临床诊疗的意义和价值。方法选取该院2018年6月—2020年6月期间收治的36例2型糖尿病合并乳腺癌患者和35例未合并2型糖尿病的乳腺癌患者,分别作为观察组和对照组,通过免疫组化检测,对比两组患者的ER、PR、Her-2、Ki-67表达情况以及乳腺癌分子分型。结果与对照组相比,观察组的ER阳性表达率(47.22%<71.43%)、PR阳性表达率(52.78%<77.14%)以及乳腺癌分子分型为LuminalA型的患者比例(22.22%<51.43%)、LuminalB型的患者比例(16.67%<42.86%)相对更低,差异有统计学意义(χ^(2)=4.304、4.618、6.119、5.510,P<0.05),而Her-2阳性表达率(63.68%>34.29%)、Ki-67阳性表达率(52.78%>28.57%)以及乳腺癌分子分型为HER2型的患者比例(27.78%>2.86%)、Basal-Like型的患者比例(30.56%>2.86%)相对更高,差异有统计学意义(χ^(2)=6.222、4.304、8.737、10.057,P<0.05)。结论在2型糖尿病合并乳腺癌患者中,根据ER、PR、Her-2、Ki-67在肿瘤组织中的表达,能够了解2型糖尿病对于乳腺癌发生、进展的影响。乳腺癌患者的治疗和护理过程中,注意2型糖尿病的防治,有效控制血糖的稳定,有助于改善患者的预后。 Objective To investigate the significance and value of the expression of ER,PR,Her-2,Ki-67 in tumor tissues for the clinical diagnosis and treatment of type 2 diabetes malignant breast cancer.Methods 36 patients with type 2 diabetes and breast cancer and 35 patients with breast cancer without type 2 diabetes who were admitted to the hospital from June 2018 to June 2020 were selected as the observation group and control group,and the immunization chemical testing was used to compare the expression of ER,PR,Her-2,Ki-67 and the molecular typing of breast cancer in the two groups.Results Compared with the control group,the positive expression rate of ER in the observation group(47.22%<71.43%),the positive expression rate of PR(52.78%<77.14%)and the molecular type of breast cancer were Luminal A of patients with type B(22.22%<51.43%),the proportion of patients with Luminal B type(16.67%<42.86%)was relatively lower,the difference was statistically significant(χ^(2)=4.304,4.618,6.119,5.510,P<0.05),and the positive expression rate of Her-2(63.68%>34.29%),Ki-67 positive expression rate(52.78%>28.57%)and the proportion of patients whose breast cancer molecular type is HER2(27.78%>2.86%);the proportion of patients with Basal-Like type(30.56%>2.86%)was relatively higher,the difference was statistically significant(χ^(2)=6.222,4.304,8.737,10.057,P<0.05).Conclusion In patients with type 2 diabetes and breast cancer,according to the expression of ER,PR,Her-2,and Ki-67 in tumor tissues,it is possible to understand the impact of type 2 diabetes on the occurrence and progression of breast cancer.In the treatment and care of breast cancer patients,attention should be paid to the prevention and treatment of type 2 diabetes,which can effectively control the stability of blood glucose and help improve the prognosis of patients.
