腹腔镜下远端胃癌根治术与传统开腹手术治疗早期胃癌的效果比较

Comparison of efficacy between laparoscopic radical distal gastrectomy and traditional open surgery in the treatment of early gastric cancer

目的比较腹腔镜下远端胃癌根治术与传统开腹手术治疗早期胃癌的临床效果。方法将2010年9月至2019年8月我院收治的80例早期胃癌患者随机分为对照组(n=40,传统开腹手术)和观察组(n=40,腹腔镜下远端胃癌根治术)。比较两组患者的治疗效果、手术相关指标、CRP、CA72-4水平以及术后并发症发生情况。结果观察组的治疗总有效率高于对照组,差异具有统计学意义(P<0.05)。观察组的手术时间、切口长度、术中出血量均优于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的CRP、CA72-4水平均降低,且观察组低于对照组,差异具有统计学意义(P<0.05)。观察组术后并发症总发生率低于对照组,差异具有统计学意义(P<0.05)。结论相较传统开腹手术,腹腔镜下远端胃癌根治术治疗早期胃癌的临床效果显著,不仅减少了术中出血量和手术时间,而且切口长度较小,同时安全性较高,值得临床推广应用。

Objective To compare the clinical effect of laparoscopic radical distal gastrectomy and traditional open surgery in the treatment of early gastric cancer.Methods Eighty patients with early gastric cancer admitted in our hospital from September 2010 to August 2019 were randomly divided into control group(n=40,traditional open surgery)and observation group(n=40,laparoscopic radical distal gastrectomy).The therapeutic effects,operation-related indexes,CRP and CA72-4 levels and postoperative complications were compared between the two groups.Results The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).The operation time,incision length and intraoperative blood loss in the observation group were better than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the CRP and CA72-4 levels in both groups decreased,those in the observation group were lower than the control group,and the differences were statistically significant(P<0.05).The total incidence of postoperative complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Compared with traditional open surgery,laparoscopic radical distal gastrectomy for early gastric cancer has a significant clinical effect,it not only reduces intraoperative blood loss and operation time,but also has a smaller incision length and higher safety,which is worthy of clinical promotion and application.
