快速康复外科理念在肺癌胸腔镜围术期的应用及对患者生活质量的影响

Application of fast track surgery concept in perioperative period of thoracoscopic surgery for lung cancer and its effect on quality of life of patients

目的分析快速康复外科(FTS)理念在肺癌胸腔镜围术期的应用效果。方法选取2017年1月至2018年11月于安康市中心医院行胸腔镜肺癌根治术治疗的88例患者,按照随机数字表法将其分为对照组和观察组,各44例。两组患者均择期行胸腔镜肺癌根治术治疗。对照组给予常规护理,观察组在常规护理的基础上应用FTS理念。比较两组患者的术后康复情况住院费用、疼痛评分、生活质量及并发症发生情况。结果观察组的肛门排气时间、排便时间、下床活动时间、住院时间均短于对照组,住院费用低于对照组(P<0.05)。观察组术后1、3、5 d的VAS评分均明显低于对照组(P<0.05)。观察组术后5 d的EORTC QLQ-C30各功能量表评分均高于对照组(P<0.05)。观察组的并发症总发生率为11.36%,低于对照组的29.55%(P<0.05)。结论将FTS理念应用于肺癌胸腔镜围术期可缩短术后康复时间及住院时间,缓解术后疼痛,提升患者的生活质量,降低并发症发生率。

Objective To analyze the application effect of fast track surgery(FTS)concept in the perioperative period of thoracoscopic surgery for lung cancer.Methods From January 2017 to November 2018,88 patients underwent thoracoscopic radical resection of lung cancer in Ankang central hospital were selected and divided into control group and observation group according to random number table method,with 44 cases in each group.The patients of both groups were scheduled for thoracoscopic radical resection of lung cancer.The control group was given routine nursing,and the observation group was given FTS concept on the basis of routine nursing.The postoperative rehabilitation,hospitalization cost,pain score,quality of life and complications were compared between the two groups.Results The anal exhaust time,defecation time,ambulation time and hospitalization time of the observation group were shorter than those of the control group,and the hospitalization cost of the observation group was lower than that of the control group(P<0.05).The VAS scores of the observation group were significantly lower than those of the control group at 1,3 and 5 d after operation(P<0.05).The functional scale scores of EORTC QLQ-C30 in the observation group were higher than those in the control group at 5 d after operation(P<0.05).The total incidence of complications in the observation group was 11.36%,which was lower than 29.55%in the control group(P<0.05).Conclusion FTS concept applied in the perioperative period of thoracoscopic surgery for lung cancer can shorten the postoperative recovery time and hospitalization time,relieve postoperative pain,improve the quality of life,and reduce the risk of complications of patients.
