PVP术前口服索利那新对术后膀胱痉挛的防治效果及生活质量的影响

Influence of oral administration of Solifenacin before PVP surgery on prevention and treatment effects of postoperative bladder spasm and quality of life

目的探究经尿道前列腺绿激光汽化切除术(PVP)术前口服索利那新对术后膀胱痉挛的防治效果及生活质量的影响。方法选择2019年2月—2020年2月于本院接受经尿道前列腺绿激光汽化切除术治疗的92例良性前列腺增生患者作为研究对象,随机数表法分为对照组和观察组两组,每组各46例。对照组口服安慰剂,观察组口服索利那新防治术后膀胱痉挛,比较两组患者膀胱痉挛情况、排尿情况、膀胱痉挛症状评分及生活质量差异。结果术后3天,观察组膀胱痉挛次数显著少于对照组(P<0.05),持续时间显著短于对照组(P<0.05);拔管后3天,观察组患者平均每日排尿次数、每日夜尿次数显著少于对照组(P<0.05),最大尿流率显著大于对照组(P<0.05),冲洗液转清时间、留置导尿管时间显著短于对照组(P<0.05);拔管后30天,两组患者膀胱痉挛相关症状评分[国际前列腺症状评分表(IPSS)、膀胱过度活动症评分(OABSS)及视觉模拟评分法(VAS)评分]及生活质量指数评分较术前均显著降低(P<0.05),且观察组低于对照组(P<0.05)。结论PVP术前口服索利那新能够有效防治患者术后膀胱痉挛,缓解患者膀胱痉挛症状,有助于改善患者生活质量。

Objective To explore the influence of oral administration of Solifenacin before transurethral photoselective vaporization of the prostate(PVP)on prevention and treatment effects of postoperative bladder spasm and quality of life.Methods Ninety-two patients with benign prostatic hyperplasia who were scheduled to undergo PVP in our hospital from February 2019 to February 2020 were selected as study subjects and randomly divided into control group and observation group according to random number table method,with 46 cases in each group.The patients in control group were given oral administration of placebo,and the patients in observation group were given oral administration of Solifenacin to prevent and treat postoperative bladder spasm.The bladder spasm,urination,bladder spasm symptoms scores and quality of life were compared between the two groups.Results At 3 rd day after surgery,the bladder spasm frequency in observation group was significantly less than that in control group(P<0.05),and the duration was significantly shorter than that in control group(P<0.05).At 3 days after extubation,the average daily urination frequency and daily nocturia frequency in observation group were significantly less than those in control group(P<0.05),and the maximum urinary flow rate was significantly greater than in control group(P<0.05),and the flushing fluid clearing time and indwelling catheter time were significantly shorter than those in control group(P<0.05).At 30 days after extubation,the bladder spasm-related symptoms scores[International Prostate Symptom Score(IPSS),Overactive Bladder Symptom Score(OABSS)and Visual Analogue Score(VAS)]and quality of life score in the two groups were significantly reduced compared with those before surgery(P<0.05),and the scores in observation group were lower than those in control group(P<0.05).Conclusions Oral administration of Solifenacin before PVP could effectively prevent and treat postoperative bladder spasm,relieve the symptoms of bladder spasm,and help improve the quality of life of patients.
