布比卡因不同阻滞方式对无张力疝修补术的影响研究

Study on effect of bupivacaine different block methods on tension-free hernia repair

目的比较布比卡因两种阻滞方式对无张力疝修补术的影响。方法选取2016年7月至2019年7月120例成年腹股沟疝患者为研究对象,根据不同麻醉阻滞方式分为研究组和对照组,对照组将布比卡因广泛注射于手术区域,研究组在此基础上再进行布比卡因周围神经浸润麻醉,比较两组手术时间、术后恢复时间及术后3个月术区疼痛情况。结果研究组与对照组平均手术时间[(52.0±12.5)min vs.(48.0±15.0)min]、平均住院时间[(2.8±0.5)d vs.(3.0±0.5)d]比较,差异无统计学意义(P>0.05)。术后3个月研究组视觉模拟评分法(VAS)<4分的患者比例高于对照组(90.0%vs.68.3%),差异有统计学意义(P<0.05)。结论布比卡因周围神经浸润麻醉对无张力疝修补术患者的术区疼痛影响更小。

Objective To compare the effects between two blocking methods of bupivacaine on tension-free hernia repair.Methods A total of 120 patients with adult inguinal hernia from July 2016 to July 2019 were selected as the study subjects and divided into the study group and control group according to different anesthesia block modes.The control group was extensively injected by bupivacaine into the surgical area,while the study group performed the peripheral nerve infiltration anesthesia with bupivacaine on the basis of the control group.The operation time,postoperative recovery time,and pain in the operation area in postoperative 3 months were compared between the two groups.Results There was no statistically significant difference in the average operation time and the average hospital stay between the study group and the control group[(52.0±12.5)min vs.(48.0±15.0)min,(2.8±0.5)d vs.(3.0±0.5)d,P>0.05].The proportion of the patients with the visual analogue scale(VAS)score<4 points at postoperative 3 months in the study group was higher than that in the control group(90.0%vs.68.3%),and the difference was statistically significant(P<0.05).Conclusion Bupivacaine peripheral nerve infiltration anesthesia has less effect on the pain of the operation area in the patients with tension-free hernia repair.
