血清胃蛋白酶原Ⅰ、血清胃蛋白酶原Ⅱ及比值联合Hp抗体检测对早期胃癌的诊断价值分析

Serum pepsinogenⅠ,Ⅱand joint Hp antibody detection ratio value analysis to the diagnosis of early gastric cancer

目的:分析血清胃蛋白酶原Ⅰ(PGⅠ)、血清胃蛋白酶原Ⅱ(PGⅡ)及比值(PGR)联合Hp抗体检测对早期胃癌的诊断价值。方法:选取2019年1月~2019年12月收治的80例胃部疾病患者,根据病理分为胃溃疡组22例,萎缩性胃炎组19例,早期胃癌组20例,进展期胃癌组19例,另纳入同期收治的查体健康者20例视为对照组。检测各组PGⅠ、PGⅡ水平,计算PGR,测定各组Hp抗体的表达,并进行组间比较。分析PGⅠ、PGⅡ、PGR、Hp抗体阳性率与胃癌严重程度的相关性。结果:在PGⅠ水平和PGR方面,进展期胃癌组<早期胃癌组<萎缩性胃炎组<胃溃疡组<对照组,差异有统计学意义(P<0.05);在PGⅡ水平和Hp抗体阳性率方面,进展期胃癌组>早期胃癌组>萎缩性胃炎组>胃溃疡组>对照组,差异有统计学意义(P<0.05)。Pearson相关性分析显示,PGⅠ水平、PGR与胃癌严重程度呈负相关,PGⅡ水平与胃癌严重程度呈正相关,差异有统计学意义(P<0.05)。结论:PGⅠ、PGⅡ、PGR联合Hp抗体检测应用在早期胃癌的诊断中效果明显,可为早期胃癌的筛查提供有效参考。

Objective To analyze the serum pepsinogenⅠ(PGⅠ),serum pepsinogenⅡ(PGⅡ)and ratio(PGR)Hp antibody detection value to the diagnosis of early gastric cancer.Method A total of 80 patients with gastric diseases admitted to our hospital from January to December 2019 were selected and divided into gastric ulcer group(22 cases),atrophic gastritis group(19 cases),early gastric cancer group(20 cases)and advanced gastric cancer group(19 cases)according to the pathology.Another 20 cases of physical examination who were admitted at the same time were regarded as the control group.To test each PGⅠ,PGⅡlevels,calculation of PGR,determination of groups of Hp antibody expression,and compared between groups.Analysis of PGⅠ,PGⅡ,PGR,Hp antibody positive rate and the correlation between the severity of gastric cancer.Results In terms of PGⅠlevel and PGR,adanced gastric carcinoma group<early gastric cancer group<atrophic gastritis group<gastric ulcer group<the control group,the difference was statistically significant(P<0.05);In terms of PGⅡlevel and the rate of positive of antibody of Hp,adanced gastric carcinoma group>early gastric cancer group>atrophic gastritis group>gastric ulcer group>the control group,the difference was statistically significant(P<0.05).Pearson correlation analysis showed that PGⅠlevel,PGR and negatively correlated to the severity of gastric cancer,PGⅡlevels and severity of gastric cancer were positively correlated(P<0.05).Conclusion PGⅠ,PGⅡ,PGR joint Hp antibody detection application in diagnosis of early gastric cancer effect significantly,can provide effective reference for screening of early gastric cancer.
