维持性腹膜透析患者矿物质代谢达标情况分析

Analysis of mineral metabolism in patients with maintenance peritoneal dialysis

摘要 目的:探讨维持性腹膜透析(PD)患者尿素清除指数(KT/Vurea)与血钙、血磷及全段甲状旁腺激素(iPTH)的关系,并调查我院PD患者血钙、血磷及iPTH达标现状。方法:回顾性分析在我院门诊定期随访的105例PD患者的临床资料,并根据入选标准及排除标准最终纳入59例患者,按照Kt/Vurea达标情况进行分组,比较各指标组间临床资料的差异性。结果:入选的59例PD患者Kt/Vurea达标率55.9%,与未达标组相比,达标组患者女性比例高,体重指数(BMI)偏低,残余肾功能明显优于未达标组;达标组患者血磷、钙磷乘积、iPTH水平均较未达标组偏低,差异有统计学意义(P<0.05)。结论:PD患者普遍存在钙磷代谢紊乱,充分的PD可以改善钙磷代谢,保护残余肾功能。 Objective To investigate the relationship between Urea clear ance index and serum calcium,serum phosphorus and intact parathyroid hormone(iPTH)in patients with peritoneal dialysis(PD),and to investigate the status of serum calcium,serum phosphorus and iPTH in patients with PD in our hospital.Method 105 patients who had been treated in our hospital outpatient,were retrospectively analyzed.According to the inclusion criteria and exclusion criteria,59 patients were included.The participants were divided into two groups based on their KT/Vurea.The differences of clinical data between the groups were compared.Results In the 59 patients with PD,the pass rate of KT/Vurea was 55.9%.The passed participants tended to be females and had lower BMI and better residual renal function,the levels of serum phosphorus,calcium-phosphorus product,and iPTH were lower compared with participants who failed KT/Vurea(P<0.05).Conclusion The disorder of calcium and phosphorus metabolism is common in PD patients,and dialysis adequacy can regulate the calcium and phosphorus metabolism,protect the residual renal function.
