摘要
目的调查糖代谢异常者亚临床甲状腺功能减退症(SCH)的患病率及其相关因素。方法在甘肃省内,通过多阶段分层整群随机抽样抽取2855名汉族成年居民作为研究对象。分析其糖代谢和SCH患病情况,并通过多元logistic回归分析糖代谢异常者发生SCH的相关因素。结果糖代谢正常者占76.29%,糖代谢异常者占23.71%,其中空腹血糖受损占3.54%,糖耐量减低占11.17%,糖尿病占9.00%。SCH总患病率为22.84%,女性SCH患病率(26.86%)高于男性(18.97%)(P<0.05)。糖代谢异常者SCH患病率(27.77%)高于糖代谢正常人群(21.30%)(P<0.05)。多元logistic回归分析结果显示,年龄、女性、空腹血糖、血清总胆固醇、血尿酸是糖代谢异常者发生SCH的危险因素(均P<0.05)。结论糖代谢异常者发生SCH的概率上升,年龄增长、女性、高空腹血糖、高血清总胆固醇、高血尿酸会增加糖代谢异常者继发SCH的风险。
Objective To investigate the prevalence of and factors related to subclinical hypothyroidism(SCH)in patients with abnormal glucose metabolism.Methods In Gansu province,2855 Han adult residents were randomly sampled for participation by multistage stratified cluster sampling.Glucose metabolism and SCH presence were assessed,and the related factors of SCH in abnormal glucose metabolism were analyzed by multiple logistic regression.Results Normal glucose metabolism was found in 76.29%of participants and abnormal glucose metabolism in 23.71%,among which 3.54%had impaired fasting glucose,11.7%had impaired glucose tolerance,and 9.0%were diabetic.The overall prevalence of SCH was 22.84%,and the prevalence of SCH in females(26.86%)was higher than in males(18.97%)(P<0.05).SCH was more prevalent in those with abnormal glucose metabolism(27.77%)than in those with normal glucose metabolism(21.30%)(P<0.05).Multiple logistic regression analysis showed that age,female sex,fasting blood glucose,serum total cholesterol,and serum uric acid were risk factors for SCH in abnormal glucose metabolism(all P<0.05).Conclusion Patients with abnormal glucose metabolism have an increased probability of comorbid SCH.Greater age,female sex,high fasting blood glucose,high serum total cholesterol,and high blood uric acid will increase the risk of secondary SCH.
