真实世界晚期胃癌一线化疗模式多维评估

Multidimensional evaluation of different chemotherapy regimens for advanced gastric cancer in real world

目的了解我国晚期胃癌患者接受的一线姑息化疗方案、生存情况、生活质量变化,并初步进行不同化疗模式的治疗花费分析。方法本研究为观察性研究,治疗方案由临床医生制定,收集患者的病例资料、住院费用信息并进行描述分析,采用Kaplan-Meier法、K-W检验进行生存分析和组间生活质量比较。结果共纳入455名患者,中位生存期11.5个月,治疗模式分为四种,组间中位生存期及生活质量比较均无统计学意义(均P>0.05),一线治疗平均总花费分别为39 701元、53 009元、56 495元、11 2074元。结论一线化疗模式主要为两药联合方案,以铂类联合氟尿嘧啶类方案、紫杉类联合氟尿嘧啶类方案为主。一线治疗花费与中位生存期呈正相关。一线化疗对生活质量的短期影响不明显。

Objective To understand first-line palliative chemotherapy,survival and quality of life of patients with advanced gastric cancer in China,and to analyze the cost of different chemotherapy patterns.Method This study was an observational study,and the treatment plan was formulated by clinicians.Patients’case data and hospitalization cost information were collected and analyzed by description.Kaplan-Meier test and K-W test were used for survival analysis and comparison of quality of life comparison between groups.Result A total of 455 patients were included in the full analysis set.The median survival time was 11.5 months.The treatment patterns could be classified into four groups.There was no significant difference in median survival time and quality of life between the four treatment modes(all P>0.05).The average cost of first-line treatment was 39701 yuan,53009 yuan,56495 yuan and 112074 yuan,respectively.Conclusion The first-line chemotherapy pattern is mainly two-drug combination regimen,especially platinum combined with fluorouracil regimen and taxane combined with fluorouracil regimen.The cost of first-line treatment is positively correlated with the median survival time.The short-term effect of first-line chemotherapy on quality of life was not obvious.
