慢性创面临床防治的现状与思考

Present Situation and Thinking of Clinical Prevention and Treatment of Chronic Wounds

慢性创面不仅危害个人心身健康,增加家庭经济负担,还会消耗社会医疗资源,其形成因素多而复杂,常见类型为创伤性溃疡、压迫性溃疡、代谢性溃疡、静脉性溃疡、缺血性溃疡等。临床防治需注重整体观念,改善创面局部环境,内外同治,中西医结合,去除病因,改善营养和代谢,选择适合创面愈合的外用药,同时加强临床防治的网络建设,建立临床防治的多学科诊疗模式。基因疗法、干细胞移植、内镜技术等新技术是未来研究的热点。

Chronic wounds not only harm personal physical and mental health,increase family economic burden,but also consume social medical resources.There are many complex factors for the formation of chronic wounds.The common types of chronic wounds include traumatic ulcer,compression ulcer,metabolic ulcer,venous ulcer,ischemic ulcer,and so on.Clinical prevention and treatment should pay attention to the overall concept,local environmental improvement,internal and external treatment,integrated Chinese and Western medicine,etiological removal,nutritional and metabolic amelioration,and selection of external drugs suitable for wound healing.Furthermore,the network construction of clinical prevention and treatment should be strengthened and multidisciplinary diagnosis and treatment model should be established.New technologies such as gene therapy,stem cell transplantation and endoscopy are the focus of future research.
