摘要
目的:研究鼻咽癌中miR-421和FOXO4的表达及与放疗敏感性和预后的相关性。方法:选择2013年1月-2015年12月期间在我院诊断为鼻咽癌并接受放疗的92例患者及同期经组织活检确诊的鼻咽部慢性炎症的50例患者,检测miR-421、FOXO4的表达量,评价鼻咽癌患者的放疗敏感性、随访鼻咽癌患者的远期生存。结果:鼻咽癌组织中FOXO4的阳性表达率明显低于鼻咽部慢性炎症组织,miR-421的表达明显高于鼻咽部慢性炎症组织;T_(3-4)期、N_(2-3)期、M 1期、临床Ⅲ-Ⅳ期鼻咽癌组织中miR-421的表达量明显高于T_(1-2)期、N_(0-1)期、M 0期、临床Ⅰ-Ⅱ期鼻咽癌组织,FOXO4的阳性率明显低于T_(1-2)期、N_(0-1)期、M 0期、临床Ⅰ-Ⅱ期鼻咽癌组织;放疗不敏感的鼻咽癌组织中miR-421的表达量明显高于放疗敏感的鼻咽癌组织,FOXO4的阳性率明显低于放疗敏感的鼻咽癌组织;FOXO4阳性表达的鼻咽癌患者的无进展生存期及总生存期均明显优于FOXO4阴性表达的鼻咽癌患者。结论:miR-421/FOXO4途径的改变与鼻咽癌发生、病理进程、放疗敏感性及远期预后均密切相关。
Objective:To study the expression of the miR-421 and FOXO4 in nasopharyngeal carcinoma and its correlation with radiosensitivity and prognosis.Methods:92 cases of patients diagnosed as nasopharyngeal carcinoma and received radiotherapy in our hospital from January 2013 to December 2015 and 50 patients with chronic nasopharyngeal inflammation confirmed by biopsy in the same period were selected.The expression of miR-421 and FOXO4 were determined.The radiosensitivity of nasopharyngeal cancer patients was evaluated.The long-term survival of nasopharyngeal cancer patients was followed up.Results:The positive expression rate of FOXO4 in nasopharyngeal carcinoma tissues was significantly lower than that in chronic nasopharynx inflammation tissues,and the expression of miR-421 was significantly higher than that in chronic nasopharynx inflammation tissues.The expression of miR-421 in T_(3-4),N_(2-3),M 1 and clinical stage Ⅲ-Ⅳ nasopharyngeal carcinoma tissues were significantly higher than that in T_(1-2),N_(0-1),M 0 and clinical stage Ⅰ-Ⅱ nasopharyngeal carcinoma tissues.The positive rate of FOXO4 were significantly lower than that in T_(1-2),N_(0-1),M 0 and clinical stage Ⅰ-Ⅱ nasopharyngeal carcinoma tissues.The expression of miR-421 in nasopharyngeal carcinoma tissues which were insensitive to radiotherapy was significantly higher than that in nasopharyngeal carcinoma tissues which were sensitive to radiotherapy,and the positive rate of FOXO4 was significantly lower than that in nasopharyngeal carcinoma tissues which were sensitive to radiotherapy.The progression-free survival and total survival of nasopharyngeal cancer patients with FOXO4 positive expression were significantly better than those with FOXO4 negative expression.Conclusion:The alteration of the miR-421/FOXO4 pathway is closely related to the occurrence,pathological process,radiosensitivity and long-term prognosis of nasopharyngeal carcinoma.
