螺旋CT扫描及三维重建技术在股骨颈骨折Garden分型中的应用价值 被引量:7

摘要 目的探讨螺旋CT扫描及三维重建技术在股骨颈骨折Garden分型中的应用价值。方法回顾性分析2017年9月~2018年9月于抚顺市中心医院治疗的231例股骨颈骨折患者的临床资料,依据诊断方式的不同分为试验组(n=114)和参照组(n=117)。参照组行X线正位片检查,试验组行螺旋CT扫描及三维重建技术。比较两组的准确率、误诊率、漏诊率、骨折内固定手术情况、术后随访结果。结果试验组的准确率高于参照组,且试验组的误诊率、漏诊率低于参照组,差异有统计学意义(P<0.05);试验组骨头坏死率、骨折不愈率、死亡率低于参照组,差异有统计学意义(P<0.05)。结论螺旋CT扫描及三维重建技术在股骨颈骨折Garden分型中有一定的应用价值,能够更为全面地观察患者股骨颈骨折的损伤情况,准确判定骨折的分型,有利于临床医师进行手术方案的选定,提高预后效果。 Objective To explore the application value of spiral CT scanning and three-dimensional reconstruction technology in Garden classification of femoral neck fracture.Methods Clinical data of 231 patients with femoral neck fracture treated in Fushun Central Hospital from September 2017 to September 2018 were retrospectively analyzed,and they were divided into experimental group(n=114)and reference group(n=117)according to different diagnostic methods.The reference group underwent X-ray positron radiography,while the experimental group underwent spiral CT scanning and three-dimensional reconstruction technology.The accuracy rate,misdiagnosis rate,missed diagnosis rate,internal fixation of fracture and follow-up results were compared between two groups.Results The accuracy rate of the experimental group was higher than that of the reference group,and the misdiagnosis rate and missed diagnosis rate of the experimental group were lower than those of the reference group,the differences were statistically significant(P<0.05).The rate of osteonecrosis,fracture healing rate and mortality of the experimental group were lower than those of the reference group,the differences were statistically significant(P<0.05).Conclusion Spiral CT scanning and three-dimensional reconstruction technology has application value in Garden classification of femoral neck fracture,which can more comprehensively observe the injury of patients with femoral neck fracture and accurately determine the fracture classification,which is conducive to the selection of surgical scheme for clinicians and improve the prognosis.
