摘要
目的观察银屑病患者皮损部位黑素瘤分化相关基因5(melanoma differentiation-associated gene 5,MDA5)表达及通路活化情况,并分析其与皮损面积、严重程度的相关性。方法选取本院50例银屑病患者,参照银屑病面积和严重程度指数(psoriasis area and severity index,PASI)评估其皮损面积与严重程度,取同期50例正常健康者作为对照,比较银屑病患者皮损组织、非皮损组织及正常健康者皮肤组织中MDA5、IL-23、MDA5通路下游促炎因子IL-6、TNF-α以及IFN-I表达水平,并分析其与PASI评分相关性。结果MDA5、IL-23 mRNA表达及MDA5通路下游IL-6、TNF-α及IFN-I mRNA表达为:皮损组织>非皮损组织>正常者皮肤组织,组间两两比较差异有统计学意义(P<0.017);MDA5、IL-23蛋白免疫印迹(Western blot,WB)数值为:皮损组织>非皮损组织>正常者皮肤组织,组间两两比较差异有统计学意义(P<0.017);MDA5、IL-23、IL-6、TNF-α及IFN-I mRNA表达与PASI评分呈正相关(P<0.05)。结论银屑病患者皮损部位MDA5、IL-23表达呈现显著升高趋势,MDA5通路下游促炎因子表达明显上调,通路活化增加,且和皮损面积、严重程度呈显著正相关性。
Objective To observe the expression of melanoma differentiation-associated gene 5(MDA5)and pathway activation in skin lesions of patients with psoriasis,and to analyze their correlation with the area and severity of skin lesions.Methods A total of 50 patients with psoriasis treated in our hospital were selected,and the area and severity of skin lesions were evaluated with psoriasis area and severity index(PASI).Meanwhile,50 healthy persons were selected as the controls.Expression levels of MDA5,IL-23,downstream pro-inflammatory factors of the MDA5 pathway IL-6,TNF-αand IFN-I in psoriatic tissues,non-psoriatic tissues and healthy skin tissues were compared,and their correlation with PASI score was analyzed.Results The MDA5 and IL-23 mRNA expression,downstream IL-6,TNF-αand IFN-I mRNA expression of the MDA5 pathway were the highest in psoriatic tissues,followed by non-psoriatic tissues,and normal skin tissues,with statistically significant intergroup differences(P<0.05).Western blot(WB)values of MDA5 and IL-23 were the highest in psoriatic tissues,followed by non-psoriatic tissues,and normal skin tissues,with statistically significant intergroup differences(P<0.017).The mRNA expression of MDA5,IL-23,IL-6,TNF-αand IFN-I was positively correlated with the PASI score(P<0.05).Conclusion The expression of MDA5 and IL-23 in skin lesions of patients with psoriasis shows a significantly increasing trend.The expression of downstream pro-inflammatory factors of the MDA5 pathway is significantly up-regulated and the pathway activation is increased,which are significantly and positively correlated with the area and severity of lesions.
