HIF-1α的表达水平与前列腺癌术后进展风险的关系研究

摘要 目的探讨缺氧诱导因子1α(HIF-1α)的表达水平与前列腺癌术后进展风险的关系。方法 90例前列腺疾病住院患者,根据前列腺组织性质不同分为A组(经尿道前列腺电切术留取的石蜡包埋良性前列腺增生组织)和B组(经前列腺癌根治术留取的石蜡包埋前列腺癌组织),每组45例。采用免疫组织化学染色方法测定两组的HIF-1α蛋白表达水平。比较A组、B组HIF-1α蛋白阳性检出率;比较不同术后临床病理特征前列腺癌患者HIF-1α表达情况。结果 HIF-1α在前列腺术后组织中均有表达, B组HIF-1α蛋白阳性检出率为77.78%,明显高于A组的13.33%,差异具有统计学意义(P<0.05)。Gleason分级低分化患者HIF-1α阳性率95.00%高于高、中级分化患者的64.00%, TNM分期Ⅲ~Ⅳ期患者HIF-1α阳性率88.89%高于Ⅰ~Ⅱ期患者的61.11%,淋巴结转移患者HIF-1α阳性率100.00%高于无淋巴结转移患者的65.52%,癌穿透被膜患者HIF-1α阳性率94.44%高于无癌穿透被膜患者的66.67%,差异均具有统计学意义(P<0.05)。结论前列腺癌术后HIF-1α在前列腺癌组织中表达增强,其可能参与疾病的发生及发展过程, HIF-1α表达增强可能与前列腺癌术后患者病情恶化有关,提示患者预后不佳。 Objective To discuss the correlation between the expression of hypoxia inducible factor-1α(HIF-1α) and the risk of postoperative progression of prostate cancer. Methods A total of 90 inpatients with prostate cancer were divided into group A(paraffin-embedded benign prostatic hyperplasia tissue from transurethral resection of the prostate) and group B(paraffin-embedded prostate cancer tissue from radical prostatectomy) according to the nature of the gland tissue, with 45 cases in each group. Immunohistochemical staining method was used to detect the expression level of HIF-1α protein. Comparison was made on positive rate of HIF-1α protein between group A and group B, and expression of HIF-1α in prostate cancer patients with different postoperative clinicopathological characteristics. Results HIF-1α was expressed in tissues after prostate surgery. The positive rate of HIF-1α protein of group B was 77.78%, which was obviously higher than 13.33% of group A, and the difference was statistically significant(P<0.05). The positive rate of HIF-1α in patients with low differentiated Gleason classification was 95.00%, which was higher than 64.00% of patients with high and intermediate differentiation;the positive rate of HIF-1α in patients with TNM stage Ⅲ-Ⅳwas 88.89%, which was higher than 61.11% in patients with TNM stage Ⅰ-Ⅱ;the positive rate of HIF-1α in patients with lymph node metastasis was 100.00%, which was higher than 65.52% in patients without lymph node metastasis;the positive rate of HIF-1α in patients with cancer penetrating capsule was 94.44%, which was higher than 66.67% in patients without cancer penetrating capsule. All the difference was statistically significant(P<0.05). Conclusion After prostate cancer surgery, the expression of HIF-1α is increased in prostate cancer tissues, HIF-1α may be involved in the occurrence and development of the disease. The increased expression of HIF-1α may be related to the worsening of the patient’s condition after prostate cancer surgery, suggesting a poor prognosis.
