癌胚抗原、甲胎蛋白、糖类抗原199、153/125及CA724在胃癌中的表达研究

Study on expression of serum CEA,AFP,CA199,CA72-4 in gastric cancer

目的研究胃癌患者血清中的甲胎蛋白(AFP)、癌胚抗原(CEA),糖类抗原199(CA199)、153(CA153)、125(CA125)以及724(CA724)等指标病理检查的临床意义。方法选取2016年1月—2019年6月济源市人民医院进行病理检查疑似为胃癌的患者30例将其设定为研究组,将同期确诊为良性胃黏膜组织病变的30例患者设定为参照组,以相同方式对所有患者血清中的肿瘤标志物表达进行检测,对比结果。结果检测结查显示,研究组患者血清中的多项肿瘤标志物的阳性表达率均明显高于参照组,对比差异有统计学意义(P<0.05);采用联合检测血清肿瘤标志物的灵敏度为86.67%,显著高于各个单项项目检测,对比差异有统计学意义(P<0.05);病理学检查联合多项肿瘤标志物检测胃癌的准确率为96.67%,明显高于单独采用病理学检查准确率(53.33%),对比差异有统计学意义(P<0.05)。结论采用血清中多项肿瘤标志物和病理学两种方式对胃癌患者进行疾病诊断,其诊断的准确性更高,可有效降低临床漏诊率,为患者的后期治疗提供可靠依据,值得在临床上大力推广使用。

Objective To study the clinical significance of alpha-fetoprotein(AFP),carcinoembryonic antigen(CEA),carbohydrate antigen 199(CA199),153(CA153),125(CA125)and 724(CA724)in serum of patients with gastric cancer.Methods From January 2016 to June 2019,in our hospital for pathologic examination suspected as 30 cases of gastric cancer patients to set it for the team,will be diagnosed with benign gastric mucosa tissue pathological changes of 30 patients were set as control group,in the same way for all of the patients in the serum tumor markers were detected,compared the results.Results The results showed that the positive expression rate of several tumor markers in serum of the study group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).The sensitivity of combined detection of serum tumor markers was 86.67%,significantly higher than that of each single item(P<0.05).The accuracy of pathological examination combined with multiple tumor markers in detecting gastric cancer was 96.67%,significantly higher than that of pathological examination alone(53.33%),and the difference was statistically significant(P<0.05).Conclusion The diagnosis of gastric cancer patients with multiple tumor markers and pathology in the serum is more accurate,can effectively reduce the rate of clinical missed diagnosis,and provides reliable basis for the later treatment of patients,which is worthy to be widely used in clinical practice.
