摘要
目的观察窄带成像技术(NBI)联合超声微探头(UMP)对胃癌和胃癌前病变的临床诊断中价值。方法选取河南大学第一附属医院2018年2月—2019年3月60例疑似胃癌患者,均行NBI与UMP检查,以术后病理检查为"金标准",比较NBI、NBI联合UMP诊断准确性、灵敏度、特异度。结果病理检查显示胃癌42例,TNM分期为;I期10例,II期12例,III期16例,IV期4例;NBI检查灵敏度83.33%(35/42),特异度77.78%(14/18),准确性81.67%(49/60),Kappa值0.58。NBI联合UMP检查灵敏度95.24%(40/42),特异度94.44%(17/18),准确性95.00%(57/60),Kappa值0.88。NBI与NBI联合UMP诊断胃癌和胃癌前病变灵敏度、特异度比较无显著差异(P>0.05);NBI联合UMP检查准确性(95.00%)明显高于单一NBI(81.67%)(P<0.05)。结论NBI联合UMP可有效提高胃癌和胃癌前病变临床诊断准确性,对提高诊断准确性具有重要意义,可有效指导胃癌筛查及患者临床治疗,具有较高应用价值。
Objective To observe the value of narrow-band imaging(NBI)combined with ultrasound miniature probe(UMP)in the clinical diagnosis of gastric cancer and gastric precancerous lesions.Methods Sixty patients with suspected gastric cancer who were admitted to the hospital from February 2018 to March 2019 were selected.NBI and UMP examinations were performed.With postoperative pathological examination as the golden standard,the accuracy,sensitivity and specificity was compared between NBI and NBI combined with UMP.Results The pathological examination showed that the TNM stage was 42 cases of gastric cancer.There were 10 cases of stage I,12 cases of stage II,16 cases of stage III and 4 cases of stage IV.The sensitivity of NBI examination was 83.33%(35/42),specificity was 77.78%(14/18),accuracy was 81.67%(49/60),Kappa value was 0.58.NBI combined with UMP showed sensitivity of 95.24%(40/42),specificity of 94.44%(17/18),accuracy of 95.00%(57/60),Kappa value of 0.88.There was no significant difference in sensitivity and specificity between NBI and NBI combined with UMP in the diagnosis of gastric cancer and gastric precancerous lesions(P>0.05).The accuracy of NBI combined with UMP examination(95.00%)was significantly higher than that of single NBI(81.67%)(P<0.05).Conclusion NBI combined with UMP can effectively improve the accuracy of clinical diagnosis of gastric cancer and gastric precancerous lesions,which is of great significance for improving the accuracy of diagnosis and can effectively guide the screening and clinical treatment of gastric cancer.It has high application value.
