血清S100β、CysC及IL-8与脑小血管病患者认知功能障碍的相关性

Correlation between serum S100β,CysC,IL-8 and cognitive impairment in patients with small cerebral vascular disease

目的探讨血清S100β、胱抑素C(CysC)及白细胞介素-8(IL-8)水平与脑小血管病(Cerebral small vessel disease,CSVD)患者认知功能障碍的相关性。方法选取2017年1月~2020年1月邢台市第三医院107例CSVD患者根据蒙特利尔认知评估(MoCA)评分分为认知功能障碍组(CI组,42例)和无认知功能障碍组(non-CI组,65例)。对比两组血清S100β、CysC及IL-8水平差异,分析血清S100β、CysC、IL-8水平与MoCA评分的相关性,探讨各指标对认知功能障碍的诊断价值。结果WML+LI组血清S100β、CysC及IL-8水平显著高于WML组和LI组,CI组患者血清S100β、CysC及IL-8水平明显高于Non-CI组(P<0.05)。CI组患者血清S100β、CysC、IL-8水平与MoCA评分均呈显著负相关(P<0.05)。血清S100β、CysC、IL-8水平升高是CSVD患者发生认知功能障碍的独立危险因素(P<0.05)。血清S100β、CysC、IL-8及联合检测诊断CSVD伴认知障碍的受试者工作特征曲线(ROC)曲线下面积(AUC)为0.855、0.828、0.867、0.942。结论血清S100β、CysC、IL-8表达与CSVD患者认知障碍有关,联合检测可为认知功能障碍的诊断提供参考。

Objective To explore the correlation between S100β,Cystatin C(CysC),Interleukin-8(IL-8)levels and cognitive impairment in patients with cerebral small vessel disease(CSVD).Methods 107 CSVD patients were divided into the cognitive impairment group(CI group,42 cases)and the non-cognitive impairment group(non-CI group,65 cases)according to the Montreal cognitive assessment(MoCA)score.The serum levels of S100β,CysC and IL-8 were compared between the two groups.The correlation between serum S100β,CysC,IL-8 levels and MoCA score were analyzed.The diagnostic value of each index for cognitive impairment was explored.Results The serum levels of S100β,CysC and IL-8 in the WML+LI group were significantly higher than those in the WML group and LI group,and the levels of S100β,CysC and IL-8 in the CI group were significantly higher than those in the non-CI group(P<0.05).The serum levels of S100β,CysC and IL-8 were significantly negatively correlated with MoCA score in CI group(P<0.05).The serum S100β,CysC and IL-8 levels were independent risk factors for cognitive impairment in CSVD patients(P<0.05).The area under ROC curve(AUC)of serum S100β,CysC and IL-8 and combined detection in diagnosis of CSVD with cognitive impairment were 0.855,0.828,0.867 and 0.942 respectively.Conclusion SSerum S100β,CysC and IL-8 are correlated with cognitive impairment in CSVD patients.Combined detection is helpful for the diagnosis of cognitive impairment.
