孕中期血脂LDL-C/HDL-C及TG/HDL-C比值与妊娠期糖尿病的关系

Correlation between LDL-C/HDL-C and TG/HDL-C ratios of blood lipids in the second trimester and gestational diabetes mellitus

目的分析孕中期血脂低密度脂蛋白胆固醇(LDL-C)/高密度脂蛋白胆固醇(HDL-C)及甘油三酯(TG)/HDL-C比值与妊娠期糖尿病的关系,旨在提高妊娠期糖尿病的病情评估。方法598例孕中期孕妇,根据葡萄糖耐量试验结果分为正常组(402例)和妊娠期糖尿病组(196例)。抽取静脉血检测两组孕妇的孕中期血脂相关指标。对比两组孕妇各血脂指标情况,分析妊娠期糖尿病的影响因素。结果两组孕妇的TG、HDL-C、LDL-C、LDL-C/HDL-C、TG/HDL-C比较,差异均具有统计学意义(P<0.05);两组孕妇的总胆固醇(TC)、脂蛋白a、载脂蛋白AI、载脂蛋白B、TC/HDL-C比较,差异无统计学意义(P>0.05)。Logistic回归分析显示,TG、HDL-C、LDL-C/HDL-C、TG/HDL-C是妊娠期糖尿病的影响因素(OR=1.415、0.562、1.360、1.182,P<0.05)。结论妊娠期糖尿病孕妇脂代谢存在失衡,且孕中期血脂LDL-C/HDL-C及TG/HDL-C比值是妊娠期糖尿病的影响因素,可作为妊娠期糖尿病病情变化有价值的评估指标,值得推广应用。

Objective To analyze the correlation between low density lipoprotein cholesterol(LDL-C)/high density lipoprotein cholesterol(HDL-C)and triglyceride(TG)/HDL-C ratios of blood lipids in the second trimester and gestational diabetes mellitus,so as to improve the the evaluation of gestational diabetes mellitus.Methods According to the results of glucose tolerance test,598 pregnant women in the second trimester were divided into normal group(402 cases)and gestational diabetes mellitus group(196 cases).Venous blood samples were collected to detect the related indicators of blood lipid in the second trimester of pregnancy.The blood lipid indexes of the two groups were compared,and the influencing factors of gestational diabetes mellitus were analyzed.Results There was statistically significant difference in TG,HDL-C,LDL-C,LDL-C/HDL-C and TG/HDL-C between the two groups(P<0.05).There was no statistically significant difference in total cholesterol(TC),lipoprotein a,apolipoprotein AI,apolipoprotein B,TC/HDL-C between the two groups(P>0.05).Logistic regression analysis showed that TG,HDL-C,LDL-C/HDL-C,TG/HDL-C were influencing factors of gestational diabetes mellitus(OR=1.415,0.562,1.360,1.182,P<0.05).Conclusion There is an imbalance of lipid metabolism in pregnant women with gestational diabetes mellitus,and the ratio of LDL-C/HDL-C and TG/HDL-C in the second trimester of pregnancy are influencing factors of gestational diabetes mellitus,which can be used as valuable evaluation indexes for the changes of gestational diabetes mellitus,which is worthy of promotion and application.
