急性缺血性脑卒中患者血清S100-β、Galectin-3与脑梗死体积及神经功能缺损程度的关系 被引量:14

Relationship between serum S100-β,Galectin-3,and cerebral infarction volume,neurological deficit degree in patients with acute ischemic stroke

摘要 目的探讨急性缺血性脑卒中患者血清中枢神经特异性蛋白(S100-β)、半乳凝集素-3蛋白(Galectin-3)与脑梗死体积及神经功能缺损程度的关系。方法回顾性分析2018年2月至2020年2月陕西省第二人民医院收治的120例急性缺血性脑卒中患者资料,根据患者脑梗死体积的大小将其分为大梗死组(n=38)、中梗死组(n=45)和小梗死组(n=37),根据患者神经功能缺损程度将其分为重度组(n=36)、中度组(n=43)和轻度组(n=41)。比较大梗死组、中梗死组与小梗死组,重度组、中度组与轻度组的一般资料,对影响急性缺血性脑卒中患者脑梗死体积及神经功能缺损程度的相关因素进行Logistic回归分析,采用Pearson法分析患者血清S100-β、Galectin-3水平与脑梗死体积及神经功能缺损程度的相关性。结果大梗死组、中梗死组和小梗死组患者的年龄、性别、吸烟史、饮酒史、糖尿病史、高血压史、冠心病史、甘油三酯(TG)、总胆固醇(TC)、心率(HR)、24 h平均收缩压(24 h SBP)、24 h平均舒张压(24 h DBP)比较差异均无统计学意义(P>0.05);大梗死组血清S100-β、Galectin-3高水平患者占比(32/38,33/38)大于中梗死组(22/45,24/45)与小梗死组(11/37,10/27),差异均有统计学意义(P<0.05);重度组、中度组与轻度组患者年龄、性别、吸烟史、饮酒史、糖尿病史、高血压史、冠心病史、TG、TC、HR、24 h SBP、24 h DBP比较差异均无统计学意义(P>0.05);重度组的血清S100-β、Galectin-3高水平患者占比(34/36,32/36)大于中度组(21/43,23/43)与轻度组(10/41,12/41),差异均有统计学意义(P<0.05);Logistic回归分析结果显示,S100-β、Galectin-3是急性缺血性脑卒中患者脑梗死体积及神经功能缺损程度独立影响因素(OR=1.042、1.413、2.280、2.307,P<0.05);相关性分析结果显示,血清S100-β、Galectin-3水平与急性缺血性脑卒中患者脑梗死体积及神经功能缺损程度呈明显相关性(r=0.038、0.238、0.729、0.675,P<0.05)。结论血清S100-β、Galectin-3水平的高低与急性缺血性脑卒中患者脑梗死体积及神经功能缺损程度密切相关,且其水平越高,患者的脑梗死体积越大,神经功能缺损程度越严重。 Objective To explore the relationship between serum central nerve specific protein(S100-β),galectin-3(Galectin-3),and cerebral infarction volume,neurological deficit degree in patients with acute ischemic stroke(AIS).Methods The data of 120 AIS patients admitted to Shaanxi Second People’s Hospital from February 2018 to February 2020 were retrospectively analyzed.According to different cerebral infarction volume,they were divided into large infarction group(n=38),medium infarction group(n=45),and small infarction group(n=37).According to different neurological deficit degree,they were divided into severe group(n=36),moderate group(n=43),and mild group(n=41).The general data among large infarction group,medium infarction group,and small infarction group,as well as among severe group,moderate group and mild group were compared.The influencing factors of cerebral infarction volume and neurological deficit degree were analyzed by Logistic regression analysis.The correlation between serum S100-β,Galectin-3 levels,and cerebral infarction volume,neurological deficit degree was analyzed by Pearson.Results There was no significant difference in age,gender,smoking history,drinking history,diabetes history,hypertension history,history of coronary heart disease,triglyceride(TG),total cholesterol(TC),heart rate(HR),24 h mean systolic blood pressure(24 h SBP),and 24 h mean diastolic blood pressure(24 h DBP)among large infarction group,medium infarction group,and small infarction group(P>0.05).The proportions of cases with high levels of S100-βand Galectin-3 in large infarction group(32/38,33/38)were higher than those in medium infarction group(22/45,24/45)and small infarction group(11/37,10/27),with statically significant differences(P<0.05).There was no significant difference in age,gender,smoking history,drinking history,diabetes history,hypertension history,history of coronary heart disease,TG,TC,HR,24 h SBP,and 24 h DBP among severe group,moderate group and mild group(P>0.05).The proportions of cases with high levels of S100-βand Galectin-3 in severe group(34/36,32/36)were higher than those in moderate group(21/43,23/43)and mild group(10/41,12/41),with statically significant differences(P<0.05).Logistic regression analysis showed that S100-βand Galectin-3 were independent influencing factors of cerebral infarction volume and neurological deficit degree in AIS patients(OR=1.042,1.413,2.280,2.307,P<0.05).Correlation analysis showed that levels of serum S100-βand Galectin-3 were significantly correlated with cerebral infarction volume and neurological deficit degree(r=0.038,0.238,0.729,0.675,P<0.05).Conclusion The levels of serum S100-βand Galectin-3 are closely related to the cerebral infarction volume and neurological deficit degree in AIS patients.The higher their levels,the larger the cerebral infarction volume and the more severe the neurological deficit.
