丁苯酞注射液联合银杏叶提取物治疗rt-PA溶栓后急性脑梗死的疗效和安全性研究 被引量:21

摘要 目的观察丁苯酞注射液联合银杏叶提取物治疗rt-PA溶栓后急性脑梗死患者的预后及安全性。方法选取2017-04—2020-07天津医科大学总医院滨海医院收治的急性脑梗死患者120例作为研究对象,于4.5 h内进行rt-PA溶栓治疗,随机将120例患者分为观察组60例和对照组60例。采用NIHSS评分及Barthel指数(BI)评估2组患者溶栓前和溶栓后神经功能缺损程度和生活能力,观察临床总有效率,统计治疗期间主要部位出血及不良反应发生率。结果观察组临床总有效率显著高于对照组(P<0.05),溶栓后7 d、14 d NIHSS评分、BI均较对照组好转(P<0.05);2组患者不良反应事件发生率差异无统计学意义(P>0.05)。结论急性脑梗死患者溶栓后给予银杏叶提取物联合丁苯酞氯化钠注射液治疗能改善神经功能及预后,对出血转化(HT)及其他不良反应发生率无影响。 Objective To observe the prognosis and safety of butylphthalide injection combined with ginkgo biloba extract in the treatment of acute cerebral infarction after rt-PA thrombolysis.Methods A total of 120 patients with acute cerebral infarction admitted to our department from April 2017 to July 2020 were selected as study subjects,and received rt-PA thrombolytic therapy within 4.5 h.After that,120 patients were randomly divided into observation group(60 cases) and control group(60 cases).The NIHSS score and Barthel index(BI) were used to evaluate the degree of neurological impairment and living ability of patients in both groups before and after thrombolysis.The total clinical response rate of the patients was observed,and the incidence of bleeding and adverse reactions in major sites during treatment were counted.Results The total effective rate in the observation group was significantly higher than that in the control group(P<0.05).The NIHSS score and BI of the observation group were improved compared with the control group at 7 and 14 days after thrombolysis(P<0.05).There was no statistical significance in the occurrence of adverse events between the two groups(P>0.05).Conclusion Ginkgo biloba extract combined with butylphthalide sodium chloride injection after thrombolysis in patients with acute cerebral infarction can improve neurological function and prognosis,and has no effect on the incidence of haemorrhagic transformation(HT) and other adverse reactions.
